Analysis on risk factors of dermatomyositis/polymyositis complicated with tumor

目的回顾性总结皮肌炎(dermatomyositis,DM)或多发性肌炎(polymyositis,PM)患者的临床特征,分析DM/PM合并肿瘤的独立危险因素,协助临床诊治及判断预后。方法回顾性分析在河北医科大学第二医院初次就诊并符合2004年国际肌病协作组建议的特发性炎症性疾病(IIM)分类诊断标准的212例DM/PM患者的临床资料,根据是否合并肿瘤分为合并恶性肿瘤组和未合并恶性肿瘤组,分析组间患者的临床特点及实验室检查的差异,总结DM/PM合并肿瘤的独立危险因素。结果年龄>40岁、伴有吞咽困难、向阳疹、"披肩征"、"V领征"、CA125升高、CA199升高为DM/PM合并肿瘤的独立危险因素(P<0.05)。结论发病年龄>40岁、向阳疹、"披肩征"、"V领征"、吞咽困难、CA125升高、CA199升高为DM/PM合并肿瘤的独立危险因素。

Objective To retrospectively summarize clinical characteristics of patients with dermatomyositis(DM)or polymyositis(PM),to analyze the independent risk factors of DM/PM complicated with tumor,and to present the assistance for clinical diagnosis,treatment and prognosis.Methods Clinical data of 212 DM/PM patients receiving first consultations in the Second Hospital of Hebei Medical University and being eligible for IIM classification and diagnosis criteria recommended by international myopathy cooperation group in 2004 were retrospectively analyzed.According to whether the tumor was merged,the patients were divided into the complicated malignant tumor group and non-complicated malignant tumor group,the differences in clinical characteristics of patients and laboratory examination of patients two groups were analyzed,and the independent risk factors of DM/PM were summarized.Results The age greater than 40 years old as well as accompanied dysphagia,heliotrope sign,"shawl sign","v-neck sign",elevated CA125,and elevated CA199 were found to be independent risk factors of DM/PM complicated with malignant tumor(P<0.05).Conclusion The age greater than 40 years old,heliotrope sign,"shawl sign","V neck sign",dysphagia,elevated CA125 and elevated CA199 are considered to be independent risk factors of DM/PM complicated with malignant tumor.
