术中保温在老年胃癌根治术中的应用

Intra-operative heat preservation in operations of radical gastrectomy for senile gastric cancer

目的探讨术中保温在老年胃癌根治手术患者中的应用效果。方法选取我院收治的65岁以上胃癌患者100例,随机分为两组,每组50例。对照组采用常规保温方法,观察组在常规保温措施基础上,加热术中冲洗液、血液制品及使用电热毯复合保温措施。监测两组入室时间、麻醉30min、术中60min、术中90min、手术结束的体温变化、麻醉苏醒时间、拔管时间、术中出血量及住院时长。结果观察组麻醉后30 min、术中60 min、术中90min、手术结束口腔部温度均显著高于对照组(P<0.05),观察组术中低体温率低于对照组(P<0.05),观察组清醒时间、拔管时间、出院时间均短于对照组,术中出血量少于对照组(P<0.05)。结论术中保温在老年胃癌根治患者术中的应用,能有效减少低体温的发生率、缩短麻醉苏醒及拔管时间、减少术中出血。

Objective To explore the effects of intra-operative heat preservation in elderly patients receiving radical gastric cancer surgeries.Methods One hundred patients aged over 65 years old treated in the hospital were selected as the study subjects and randomly divided into two groups with 50 cases in each group.Routine heat preservation method was implemented for patients in the control group,while intra-operative flushing fluid,blood products and electric blanket composite insulation measures were added and implemented for patients in the observation group on the basis of routine heat preservation method.The change in the body temperature of patients at the entry time of operating room,30 min post-anesthesia time,60 min-intraoperative time,90 min-intraoperative and at the end of operation,wake-up anesthesia duration,extubation duration,intra-operative blood loss and length of hospital stay of patients in two groups were compared.Results In the observation group,the oral temperature at30 min post-anesthesia time,60 min-intraoperative time,90 min-intraoperative time,and the end of the operation were significantly higher than those in the control group(P<0.05).The hypothermia rate of patients in the observation group was lower than such index of patients in the control group(P<0.05),the wake-up time,extubation time and discharge duration of patients in the observation group were shorter than such indexes of patients in the control group,and the intra-operative blood loss was less than that of patients in the control group(P<0.05).Conclusion Intra-operative heat preservation applied in treating senile gastric cancer can effectively reduce the incidence of hypothermia,shorten the duration of anaesthesia recovery and extubation,and reduce intraoperative blood loss.
