骨科患者股浅静脉血栓形成52例临床分析

Clinical analysis of 52 cases of superficial femoral vein thrombosis in orthopedic patients

目的分析骨科住院患者发生股浅静脉等高危血栓的危险因素及探讨其防治对策。方法收集河北医科大学第三医院湘江院区骨科住院患者中临床及超声诊断为股浅静脉及以上血栓形成患者52例,做回顾性分析研究。结果血栓发生部位以股浅静脉血栓合并腘静脉、胫后静脉、腓静脉、小腿肌间静脉最多见,以股浅静脉血栓合并髂外及髂内静脉血栓最少见。在所有发生股浅静脉血栓的患者中多发骨折例数最多,其次为股骨干及股骨远端骨折,第三为髋部骨折。52例发生股浅静脉血栓的骨科住院患者中42例(80.77%)合并一种或多种原发病或有其它危险因素,其中输库存血、高血压和肥胖占前3位。结论具有高危因素的患者,可在围手术期的任何时期发生血栓,应引起高度重视。

Objective To analyze the risk factors high-risk thrombosis of Patients with superficial femoral vein in orthopedic hospitalized patients and discuss their prevention and treatment strategy.Methods The clinical data of 52 patients with superficial femoral vein and above thrombosis diagnosed by clinical practice and ultrasound from January 2017 to January 2018 in the orthopedic department of the Third Hospital of Hebei Medical University(Xiangjiang district)were collected and retrospectively analyzed.Results Thrombosis was most common in superficial femoral vein combined with popliteal vein,posterior tibial vein,peroneal vein,and calf intermuscular vein,while it was rare in superficial femoral vein combined with external and internal iliac vein.In all patients with superficial femoral vein thrombosis,the number of cases of multiple fractures was the largest,followed by femoral shaft,distal femoral fractures and hip fractures.Among 52 patients with superficial femoral vein thrombosis,42(80.77%)had one or more primary diseases or other risk factors of which blood transfusion,hypertension and obesity were the top three.Conclusion Thrombosis is more likely to develop in patients with high risk factors in any phase of perioperative period,which should be paid high attention to.
