Mini-CEX在临床本科实习医师形成性评价中的应用研究

Application of Mini-CEX in Formative Evaluation of Undergraduate Interns

目的探讨迷你临床演练(mini-clinical evaluation exercise,Mini-CEX)在临床本科实习医师形成性评价中的应用价值。方法随机选取2017年7月至2018年7月在河北省承德市中心医院内科系统轮转实习的2013级5年制临床医学本科实习医师80例,均进行Mini-CEX。分别于入科时、出科时进行Mini-CEX考核评估,比较2次考核结果。结果80例实习医师出科时Mini-CEX考核评估的医疗问诊技能[(6.09±0.86)分vs(4.93±1.02)分]、体格检查技能[(6.63±0.86)分vs(4.33±0.79)分]、人文关怀[(7.19±0.76)分vs(5.46±0.86)分]、临床综合诊疗能力[(6.91±0.77)分vs(5.03±0.87)分]、临床沟通能力[(6.43±0.74)分vs(4.49±0.71)分]、组织效能[(6.61±0.77)分vs(4.76±0.78)分]、整体岗位胜任力[(7.01±0.80)分vs(4.81±0.71)分]评分均高于入科时,差异有统计学意义(P<0.05)。结论Mini-CEX在临床本科实习医师形成性评价中简便、易行、可操作性强,具有应用推广价值。

Objective To explore the application value of mini-clinical evaluation exercise(Mini-CEX)in the formative evaluation of clinical undergraduate interns.Methods A total of 80 clinical undergraduate interns in Chengde Central Hospital were randomly selected from July 2017 to July 2018.All of them participated in Mini-CEX,and Mini-CEX assessments were conducted at entrance and on completion,respectively.The assessment results were compared.Results The scores of medical interview,physical examination,humanistic care,clinical judgment,communication skills,organizational effectiveness and post competence in 80 clinical undergraduate interns on completion were higher than those at the entrance[(6.09±0.86)vs(4.93±1.02),(6.63±0.86)vs(4.33±0.79),(7.19±0.76)vs(5.46±0.86),(6.91±0.77)vs(5.03±0.87),(6.43±0.74)vs(4.49±0.71),(6.61±0.77)vs(4.76±0.78),(7.01±0.80)vs(4.81±0.71),respectively].There were statistically significant difference(P<0.05).Conclusion Mini-CEX is simple and easy to perform in the formative evaluation of clinical undergraduate interns,it is worthy of application and promotion.
