血清C3、C4及IL-6检测在年龄相关性白内障临床诊断中的作用分析

Analysis of effects of serum C3,C4 and IL-6 detection in the clinical diagnosis of age-related cataract

目的分析血清补体C3、C4及白细胞介素6(IL-6)检测在年龄相关性白内障(ARC)临床诊断中的作用。方法回顾性分析2019年1月—2022年1月华西广安医院行各项检查的202例疑似ARC患者临床资料,根据患者晶状体浑浊形态和视力情况将其分为观察组109例(ARC)和对照组93例(非ARC)。比较两组血清补体C3、C4、IL-6差异,绘制受试者工作特征曲线(ROC)评估上述指标对ARC的诊断效能。根据患者晶状体浑浊部位将ARC分为皮质性白内障69例,核性白内障23例,后囊膜下白内障17例,比较3组患者上述血清指标差异。结果观察组补体C3、C4均低于对照组(P<0.05);IL-6高于对照组(P<0.05)。ROC曲线结果显示,血清补体C3、C4、IL-6对ARC患者诊断的AUC分别为0.955、0.983、0.969。3种类型ARC血清补体C3、C4比较:核性白内障<皮质性白内障<后囊膜下白内障(P<0.05);血清IL-6比较:核性白内障>皮质性白内障>后囊膜下白内障(P<0.05)。结论ARC患者血清补体C3、C4明显降低,血清IL-6明显增高,血清补体C3、C4、IL-6在ARC的诊断方面效能较佳,且上述指标与不同类型ARC也存在一定相关性。

Objective To analyze the role of serum complements C3 and C4 and interleukin-6(IL-6)detection in the clinical diagnosis of age-related cataract(ARC).Methods The clinical data of 202 patients with suspected ARC who underwent various examinations in the hospital were retrospectively analyzed between January 2019 and January 2022.According to the lens opacity and visual acuity,the patients were divided into observation group(109 cases of ARC)and control group(93 cases of non-ARC).Serum complements C3 and C4 levels were determined by immune velocity dispersion nephelometery,and serum IL-6 level was detected by radioimmunoassay.The differences in levels of serum complements C3 and C4 and IL-6 were compared between the two groups,and receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic efficiency of the above indicators on ARC.Patients with ARC were classified into 69 cases of cortical cataract,23 cases of nuclear cataract and 17 cases of posterior subcapsular cataract according to the sites of lens opacity of patients,and the differences in serum C3,C4 and IL-6 levels were compared among the three groups.Results The levels of complements C3 and C4 in observation group were lower than those in control group(P<0.05)while the level of IL-6 was higher than that in control group(P<0.05).ROC curve results showed that the AUC values of serum complements C3 and C4 and IL-6 in the diagnosis of ARC were 0.955,0.983 and 0.969 respectively.Comparison of levels of serum complements C3 and C4 among three types of ARC was shown as nuclear cataractcortical cataract>posterior subcapsular cataract(P<0.05).Conclusion The levels of serum complements C3 and C4 in patients with ARC are significantly decreased while the level of serum IL-6 is significantly increased.The levels of serum complements C3 and C4 and IL-6 can have good efficiency in the diagnosis of ARC,and the above indicators are also related to different types of ARC.
