miR-133/208对老年急性心肌梗死患者预后的预测价值 被引量:2

摘要 目的探究微小RNA133/208(miR-133/208)对老年急性心肌梗死(AMI)患者预后的预测价值。方法选择2019年3月-2021年6月滁州市第一人民医院收治的急性心肌梗死患者198例为研究组,体检健康者50例为对照组,比较两组外周血中miR-208、miR-133相对表达量,根据198例急性心肌梗死患者1年内是否发生不良心血管事件分为预后良好组(n=168)与预后不良组(n=30),比较两组外周血中miR-208、miR-133相对表达量,采用spearman相关分析miR-208、miR-133与急性心肌梗死患者预后不良的相关性,采用受试者工作特征曲线(ROC)分析miR-208、miR-133对急性心肌梗死患者预后不良的预测价值。结果研究组外周血中miR-208、miR-133相对表达量显著高于对照组(P<0.05);预后不良组外周血中miR-208、miR-133相对表达量显著高于预后良好组(P<0.05);miR-208、miR-133与急性心肌梗死患者预后不良呈正相关(P<0.05);miR-208对急性心肌梗死患者预后不良的预测价值明显优于miR-133(P<0.05);miR-208、miR-133联合检测对急性心肌梗死患者预后不良的预测价值明显优于各指标单独检测(P<0.05)。结论miR-208、miR-133在老年急性心肌梗死患者外周血中呈高表达状态,miR-208、miR-133与老年急性心肌梗死患者预后具有明显相关性,外周血miR-208、miR-133联合检测对老年急性心肌梗死患者预后具有一定评估价值,有望成为临床管理老年急性心肌梗死的分子生物标志物。 Objective To investigate the predictive value of microRNA133/208(miR-133/208)on the prognosis of elderly patients with acute myocardial infarction(AMI).Methods 198 cases of AMI(study group)and 50 cases of healthy individuals(study group)in our hospital from March 2019 to June 2021 were enrolled.Study group was further divided into two subgroups according to the absence or presence of adverse cardiovascular events within 1 year,good prognosis group(n=168)and poor prognosis group(n=30).The relative expression of miR-208 and miR-133 in peripheral blood were compared between groups and subgroups.Spearman correlation was used to analyze the correlation between miR-208 and miR-133 and poor prognosis in AMI patients.The receiver operating characteristic curve(ROC)was used to analyze the predictive value of miR-208 and miR-133 in AMI patients.Results The relative expression levels of miR-208 and miR-133 in peripheral blood of the study group were significantly higher than those in the control group(P<0.05);the relative expression levels of miR-208 and miR-133 in peripheral blood of the poor prognosis group were significantly higher than those of the good prognosis group(P<0.05);miR-208 and miR-133 were positively correlated with poor prognosis in AMI patients(P<0.05);The predictive value of miR-208 for poor prognosis in patients with acute myocardial infarction was significantly better than miR-133(P<0.05);The combined predictive value of miR-208 and miR-133 in predicting the poor prognosis of AMI patients was significantly better than that of single detection(P<0.05).Conclusion miR-208 and miR-133 are highly expressed in peripheral blood of elderly AMI patients,which have a significant correlation with the prognosis of elderly AMI patients.Combined detection of the two in peripheral blood has certain value in evaluating the prognosis of elderly AMI patients,and is expected to be molecular biomarkers for clinical management of elderly AMI patients.
