桥本甲状腺炎患者血清Tpo Ab、Tg Ab水平在诊断和分型中的应用价值

Application value of levels of serum Tpo Ab and Tg Ab in the diagnosis and classification of patients with Hashimoto’s thyroiditis

目的探究桥本甲状腺炎(HT)患者血清过氧化物酶抗体(Tpo Ab)、甲状腺球蛋白抗体(Tg Ab)水平在诊断和分型中的应用价值。方法回顾性分析2019年6月至2021年1月期间于广安市人民医院行各项检查的125例疑似HT受检者临床资料,根据临床诊断结果分为HT组95例与非HT组30例,比较两组血清Tpo Ab、Tg Ab水平,采用受试者工作特征(ROC)曲线评估上述指标对HT的诊断效能。根据HT组患者IgG4表达分型将其分为IgG4阳性亚组39例与IgG4阴性亚组56例,比较两组血清Tpo Ab、Tg Ab水平,采用ROC曲线评估上述指标对HT患者不同IgG4表达分型的评估效能。结果HT组血清Tpo Ab、Tg Ab水平均显著高于非HT组(P<0.05),上述指标诊断HT的ROC曲线AUC分别为0.893、0.873,截断值分别为506.22IU/ml、324.14IU/ml,诊断效能良好(P<0.05)。IgG4阳性亚组血清Tpo Ab、Tg Ab水平均显著高于IgG4阴性亚组(P<0.05),上述指标评估HT患者IgG4表达分型的ROC曲线AUC分别为0.794、0.857,截断值分别为643.10IU/ml、440.32IU/ml,诊断效能良好(P<0.05)。结论HT患者血清Tpo Ab、Tg Ab存在过表达情况,而IgG4表达HT患者这种过表达情况更为显著,其表达水平用于HT诊断、分型评估均可表现出良好效能,对临床HT患者诊断及分型评估具有一定参考价值。

Objective To explore the application value of levels of serum peroxidase antibody(Tpo Ab)and thyroglobulin antibody(Tg Ab)in the diagnosis and classification of patients with Hashimoto’s thyroiditis(HT).Methods The clinical data of 125 subjects with suspected HT who underwent various examinations in the hospital of Guang An City between June 2019 and January 2021 were retrospectively analyzed.According to the clinical diagnostic results,the patients were divided into 95 cases in HT group and 30 cases in non-HT group.The levels of serum Tpo Ab and Tg Ab were compared between the two groups,and receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficiency of the above indicators on HT.According to the IgG4 expression classification of patients in HT group,they were divided into 39 cases in IgG4-positive subgroup and 56 cases in IgG4-negative subgroup.Levels of serum Tpo Ab and Tg Ab were compared between the two subgroups,and ROC curve was applied to evaluate the evaluated efficiency of the above indicators on different IgG4 expression types in patients with HT.Results The levels of serum Tpo Ab and Tg Ab in HT group were significantly higher than those in non-HT group(P<0.05),and the AUC value of ROC curves of the above indicators for diagnosing HT were 0.893 and 0.873,and the cut-off values were506.22 IU/ml and 324.14 IU/ml respectively,with good diagnostic efficiency(P<0.05).The levels of serum Tpo Ab and Tg Ab were significantly higher in IgG4-positive subgroup than those in IgG4-negative subgroup(P<0.05),and the AUC values of ROC curves of the above indicators in assessing the IgG4 expression classification of patients with HT were 0.794 and 0.857,and the cut-off values were 643.10 IU/ml and 440.32 IU/ml respectively,with good diagnostic efficiency(P<0.05).Conclusion There are overexpressions of Tpo Ab and Tg Ab in serum of patients with HT,and the overexpression status are more significant in HT patients with IgG4 expression.The expression levels of the two can show good efficiency in the diagnosis and classification evaluation of HT,and have a certain reference value for the diagnosis and classification evaluation of patients with HT in clinical practice.
