家庭医生签约服务、居民就诊行为与自评健康:基于分位数回归的实证研究

Family doctor contracting services,residents’ health seeking behavior and self-perceived health:An empirical study based on quantile regression

目的探究家庭医生签约服务、居民就诊行为与自评健康关系。方法采用分位数回归分析方法,估计家庭医生签约服务政策效应在整体签约服务对象中的分布情况。结果家庭医生签约服务对在基层医疗机构就诊比例低的人群的就诊行为的边际效应显著大于在基层医疗机构就诊比例高的人群,且家庭医生签约服务更可能对健康状况较差的人群具有显著效应。结论应持续优化家庭医生签约服务对象结构,推进家庭医生签约服务有助于消除健康不平等。

Objective To explore the relationship between family doctor contracting services,residents’health seeking behavior and self-perceived health.Methods The quantile regression analysis method was used to estimate the policy effect distribution of the family doctor contracting services in the overall contracting service objects.Results The marginal effect of family doctor contracting services on the behavior of people with a low proportion of medical consultations in primary healthcare institutions was significantly greater than that of those with a high proportion of medical consultations in primary healthcare institutions,and family doctors contracting services were more likely to be significant for people with poor health.Conclusions The structure of the family doctor contracting service objects should be continuously optimized,and the promotion of family doctor contracting services can help eliminate health inequalities.
