壮医验方胃毒清治疗阳证型胴尹(慢性胃窦炎)的效果

Discussion on the effect of Chronic Antral Gastritis treating by Zhuang Medical Prescription Weiduqing

目的:观察壮医验方胃毒清治疗阳证型胴尹的临床疗效。方法:收集符合诊断标准的幽门螺杆菌(Helicobacter pylori,HP)感染引起的阳证型胴尹患者90例,随机分为壮药组(壮医验方胃毒清)、西药组(雷贝拉唑钠肠溶胶囊、胶体果胶铋胶囊、阿莫西林分散片和克拉霉素分散片)和壮药+西药组,各30例。三组疗程均为10 d。治疗后进行中医症候积分、HP根除率和复发率、胃镜下黏膜积分、不良反应发生率的统计、对比及分析。结果:①中医证候疗效方面,壮药组疗效(93.33%,28/30)与壮药+西药组的(96.55%,28/29)相当(P>0.05),但两组均显著优于西药组(72.41%,21/29)(P<0.05)。②治疗结束1个月后复查14C尿素呼气试验,壮药+西药组转阴率(93.10%,27/29)均高于壮药组(80.00%,24/30)和西药组(72.41%,21/29)(P<0.05),而壮药组与西药组的转阴率相当(P>0.05)。③治疗后3个月复查14C尿素呼气试验,壮药+西药组复发率(3.45%,1/29)均低于壮药组(10.00%,3/30)和西药组(24.13%,7/29)(P<0.05),而壮药组与西药组的复发率相当(P>0.05)。④三组治疗结束3个月后复查胃镜,壮药组(86.67%,26/30)与壮药+西药组(89.66%,26/29)胃镜下黏膜疗效相当(P>0.05),且两组的疗效均显著优于西药组(62.07%,18/29)(P<0.05)。⑤在治疗过程中,壮药组和壮药+西药组未出现严重不良反应。结论:单纯的壮医验方胃毒清治疗HP感染性慢性胃窦炎在改善临床症状、提高HP转阴率、减少HP复发率方面与西药四联疗法的效果相当,在四联方案基础上加用壮医验方胃毒清效果更佳,未见不良反应发生,具有一定的临床推广价值。

Objective:To observe the clinical efficacy of weiduqing,a tested prescription of Zhuang medicine,in the treatment of patients with Yang syndrome.Methods:Helicobacter pylori that met the diagnostic criteria were collected;90patients with Yang syndrome caused by HP infection were randomly divided into Zhuang medicine group(weiduqing,the tested prescription of Zhuang medicine),western medicine group(rabeprazole sodium enteric coated capsule,colloidal pectin bismuth capsule,amoxicillin dispersible tablet and clarithromycin dispersible tablet)and Zhuang medicine+western medicine group.The treatment course of the three groups was 10 days.After treatment,the traditional Chinese Medicine symptom score,HP eradication rate and recurrence rate,mucosal score under gastroscope and the incidence of adverse reactions were counted,compared and analyzed.Results:(1)In terms of curative effect of TCM symptoms,the curative effect of Zhuang medicine group(93.33%,28/30)was equivalent to that of Zhuang medicine+western medicine group(96.55%,28/29)(P>0.05),but both groups were significantly better than that of Western medicine group(72.41%,21/29)(P<0.05).(2)One month after treatment,the14C urea breath test was rechecked.The negative conversion rate of Zhuang medicine+western medicine group(93.10%,27/29)was higher than that of Zhuang medicine group(80.00%,24/30)and Western medicine group(72.41%,21/29)(P<0.05),while the negative conversion rate of Zhuang Medicine group and Western medicine group was the same(P>0.05).(3)14C urea breath test was rechecked 3 months after treatment.The recurrence rate of Zhuang medicine+western medicine group(3.45%,1/29)was lower than that of Zhuang medicine group(10.00%,3/30)and Western medicine group(24.13%,7/29)(P<0.05),while the recurrence rate of Zhuang medicine group and Western Medicine group was the same(P>0.05).(4)Three months after treatment,the three groups were rechecked by gastroscope.The curative effect of Zhuang medicine group(86.67%,26/30)was equivalent to that of Zhuang medicine+western medicine group(89.66%,26/29)(P>0.05),and the curative effect of the two groups was significantly better than that of Western medicine group(62.07%,18/29)(P<0.05).(5)During the treatment,there were no serious adverse events and adverse reactions in Zhuang medicine group and Zhuang medicine+western medicine group.Conclusion:weiduqing,a simple prescription of Zhuang medicine,has the same effect as the four combination therapy of Western medicine in improving the clinical symptoms,increasing the negative rate of HP and reducing the recurrence rate of HP in patients with HP infectious chronic gastric sinusitis.The effect of adding weiduqing,a prescription of Zhuang medicine,on the basis of the four combination scheme is better,there is no adverse reaction,and has a certain clinical promotion value.
