祛风止痒汤对湿热下注型肛周湿疹患者Treg/Th17免疫平衡的影响研究

Effect of Qufeng Zhiyang Decoction on immune balance of Treg/Th17 in patients with perianal eczema induced by damp and heat

目的:探究祛风止痒汤在湿热下注型肛周湿疹患者中的疗效及对Treg/Th17免疫平衡的影响。方法:收集2017年7月至2020年6月我院200例湿热下注型肛周湿疹患者,其中对照组100例,采用依巴斯汀片口服与曲安奈德益康唑乳膏外用治疗,观察组100例,采用祛风止痒汤内服治疗,药物残渣研末后外敷。比较2组患者治疗2周后的治疗疗效,治疗后3个月、6个月复发状况及治疗后不良反应发生率。比较2组患者治疗前后瘙痒程度评分、皮损严重程度EASI评分、生活质量DLQI评分、Treg/Th17细胞比率、炎症因子IL-6与TNF-α水平。结果:观察组治疗总有效率显著高于对照组,治疗后的瘙痒程度评分、EASI评分、DLQI评分显著低于对照组,治疗后观察组的Th17细胞比率显著低于对照组,Treg细胞比率显著高于对照组,IL-6与TNF-α水平在治疗后显著低于对照组,观察组3个月、6个月复发率显著低于对照组,两组不良反应发生率无显著性差异。结论:祛风止痒汤可改善肛周湿疹患者的Treg/Th17免疫平衡及炎症水平,并通过这些途径发挥出良好的治疗疗效。

Objective:To explore the effect of Qufeng Zhiyang Decoction on Treg/Th17 immune balance in patients with perianal eczema.Methods:From July 2017 to June 2020,200 patients with perianal eczema of damp heat injection type in our hospital were collected,100 cases in the control group were treated with Ebastine tablets and triamcinolone acetonide and econazole cream,and 100 cases in the observation group were treated with Qufeng Zhiyang Decoction.The therapeutic effect,pruritus score,EASI score,DLQI score of quality of life,Treg/Th17 cell ratio,IL-6 and TNF-αlevels were compared before and after treatment.Results:the total effective rate of the observation group was significantly higher than that of the control group,the pruritus score,EASI score and DLQI score after treatment were significantly lower than those of the control group,the Th17 cell ratio of the observation group was significantly lower than that of the control group,the Treg cell ratio was significantly higher than that of the control group,and the levels of IL-6 and TNF-αafter treatment were significantly lower than those of the control group.Conclusion:Qufeng Zhiyang Decoction can improve Treg/Th17 immune balance and inflammatory level in patients with perianal eczema,and play a good therapeutic effect through these ways.
