克拉霉素联合丙酸氟替卡松对慢性鼻窦炎患者鼻黏膜纤毛传输功能及炎性因子的影响 被引量:9

EFFECT OF CLARITHROMYCIN COMBINED WITH FLUTICASONE PROPIONATE ON NASAL MUCOCILIARY TRANSPORT FUNCTION AND INFLAMMATORY FACTORS IN PATIENTS WITH CHRONIC SINUSITIS

摘要 目的探讨克拉霉素联合丙酸氟替卡松对慢性鼻窦炎患者鼻黏膜纤毛传输功能及炎性因子的影响。方法选取2017年3月—2018年3月该院收治的慢性鼻窦炎患者90例,按照随机分组方法分为二组,对照组使用克拉霉素胶囊进行治疗,观察组使用克拉霉素胶囊联合丙酸氟替卡松进行治疗。分析联合治疗对患者鼻黏膜纤毛传输功能及炎性因子的影响。结果观察组患者治疗后的鼻黏膜纤毛清除率(MCC)、鼻黏膜纤毛输送率(MTR)及治疗有效率均高于对照组,差异有统计学意义(P<0.05),但二组患者药物不良反应差异无统计学意义(P>0.05);观察组患者鼻黏膜充血肿胀、回吸痰、嗅觉减退、头痛、流脓涕、鼻塞等症状、体征改善程度优于对照组(P<0.05);观察组患者治疗后的IL-4、IL-2水平均低于对照组(P<0.05),以上差异均有统计学意义。结论鼻窦炎患者使用克拉霉素联合丙酸氟替卡松进行治疗,安全性较高,疗效显著,并且能有效避免破坏鼻腔自然结构,使得鼻黏膜纤毛传输功能有所提升,对炎症因子的生成有明显的抑制作用,患者的症状、体征得到进一步改善,不良反应得到缓解,患者康复速度加快,生活质量显著提高。 Objective To investigate the effects of Clarithromycin Combined with fluticasone propionate on ciliary transport function and inflammatory factors in nasal mucosa of patients with chronic sinusitis.Methods Ninety patients with chronic sinusitis admitted from March 2017 to March 2018 were randomly divided into two groups.The control group was treated with clarithromycin capsule and the study group was treated with clarithromycin capsule and fluticasone propionate.To analyze the effect of combined therapy on nasal mucociliary transmission function and inflammatory factors.Results After treatment,MCC,MTR and effective rate of treatment in the study group were higher than those in the control group(P<0.05),the difference was statistically significant,but there was no significant difference in adverse drug reactions between the two groups(P>0.05);symptoms and signs such as congestion and swelling of nasal mucosa,sputum respiration,hypoolfaction,headache,runny abscess and nasal obstruction in the study group were better than those in the control group(P<0.05);After treatment,the levels of IL-4 and IL-2 were lower than those of the control group(P<0.05),the above differences were statistically significant.Conclusion Clarithromycin combined with fluticasone propionate is safe and effective in the treatment of chronic sinusitis.It can effectively avoid destroying the natural structure of nasal cavity,improve the transmission function of nasal mucosa cilia,inhibit the production of inflammatory factors,further improve the symptoms and signs of patients,alleviate adverse reactions,and accelerate the recovery of patients.
