摘要
目的探讨妊娠前超敏C反应蛋白(hs-CRP)对妊娠期高血压疾病(HDP)的影响。方法选取参加2006—2017年每两年一度的开滦集团健康体检且体检后在此期间首次、单胎妊娠分娩的育龄期妇女为研究队列,其中符合纳入与排除标准的观察对象共1041例。采用多因素线性回归分析妊娠前hs-CRP对妊娠妇女产前收缩压的影响;采用多因素Logistic回归分析妊娠前hs-CRP对HDP的影响。结果依妊娠前hs-CRP三分位数将观察对象分为三组,三组间HDP患病率分别为4.42%、9.69%、8.84%(P<0.05)。多因素线性回归分析显示,在校正多种混杂因素后,lnhs-CRP每增加1个单位,产前收缩压增加0.71 mmHg(95%CI:0.05~1.38)。多因素Logistic回归分析显示,在校正多种混杂因素后,与第一分位组比较,第三分位组发生HDP的风险增加了2.10倍(95%CI:1.02~4.35)。结论妊娠前高hs-CRP是HDP的危险因素。
Objective To investigate the effects of hypersensitive c-reactive protein(hs-CRP)on hypertensive disorders of pregnancy(HDP).Methods A total of one hundred and forty-one women of childbearing age who participated in the biennial physical examination of Kailuan group from 2006 to 2017 and gave birth to a single pregnancy for the first time in the same year after the physical examination were selected as study object.Multivariate linear regression analysis was used to analyze the effect of pre-pregnancy hs-CRP on prenatal systolic blood pressure in pregnant women.Multivariate logistic regression was used to analyze the effect of pre-pregnancy hs-CRP on HDP.Results The subjects were divided into three groups according to the pre-pregnancy hs-CRP tertiles,and the prevalence of HDP was 4.42%,9.69%and 8.84%,respectively(P<0.05).Multivariate linear regression analysis showed that,after adjusting for a variety of confounding factors,every 1 unit increase in lnhs-CRP resulted in a 0.71 mmHg increase in prenatal systolic blood pressure(95%CI:0.05-1.38).Multivariate Logistic regression analysis showed a 2.10-fold increased risk of HDP in the third quartile compared with the first quartile(95%CI:1.02-4.35),after adjusting for multiple confounders.Conclusion High hs-CRP before pregnancy is a risk factor for HDP.
