加味柴胡疏肝散对冠心病合并抑郁症模型大鼠的作用及机制

The effect and mechanism of Jiawei Chaihu Shugan San on a rat model of coronary heart disease combined with depression

目的探讨加味柴胡疏肝散对冠心病合并抑郁症模型大鼠的干预作用及机制。方法75只SD雄性大鼠随机分成正常组、假手术组、模型组、加味柴胡疏肝散组。采用高脂饲料喂养,慢性不可预知性温和刺激(CUMS)及左前降支冠脉结扎术造模,共6 w。观察每组宏观体征、旷场及高架十字试验、心脏彩超、心电图、心肌细胞病理改变、血流变、血清中总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、促肾上腺皮质激素(ACTH)、皮质醇(CORT)含量的变化。结果与正常组相比,模型组出现精神萎靡、毛发杂乱油腻、反抗意识弱;旷场及高架十字试验结果显示,模型大鼠活跃程度显著下降,表现出抑郁样改变(P<0.01,P<0.05);心脏彩超左室射血分数明显下降(P<0.01);心电图示心肌缺血并伴有心律不齐;苏木素-伊红(HE)染色显示,心肌细胞结构紊乱、炎性渗出及纤维化样改变;全血黏度及血清TC、TG、LDL-C、ACTH、CORT、HDL-C水平明显升高(P<0.01)。药物组上述指标得到不同程度改善。结论加味柴胡疏肝散具有抗抑郁、改善心肌损伤、降血脂及血液黏度的作用。其中,对抑郁症状的改善考虑与血清中ACTH、CORT的含量有关。

Objective To explore the intervention effect and mechanism of Jiawei Chaihu Shugan San on a rat model of coronary heart disease combined with depression.Methods 75 SD male rats were randomly divided into normal group,sham surgery group,model group,and Jiawei Chaihu Shugan San group.Rats were fed with high-fat diet,chronic unpredictable mild stimulation(CUMS)and left anterior descending coronary artery ligation were performed for a total of 6 w.Macroscopic signs,open field and elevated cross tests,cardiac ultrasound,electrocardiogram,pathological changes in myocardial cells,hemorheology,changes of serum total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),adrenocorticotropic hormone(ACTH),and cortisol(CORT)contents in each group were observed.Results Compared with the normal group,the model group showed signs of mental fatigue,messy and greasy hair,and weak resistance consciousness;results of open field and elevated cross tests showed that the activity level of the model rats were significantly decreased,exhibiting depressive like changes(P<0.01,P<0.05);cardiac ultrasound ejection fraction was significantly decreased(P<0.01);electrocardiogram showed myocardial ischemia accompanied by arrhythmia;hematoxylin eosin(HE)staining showed structural disorder,inflammatory exudation,and fibrosis of myocardial cells change in appearance;the whole blood viscosity and serum levels of TC,TG,LDL-C,ACTH,CORT,and HDL-C were significantly increased(P<0.01).The above indicators in the drug group had improved to varying degrees.Conclusions Jiawei Chaihu Shugan San has the effects of antidepressant,improving myocardial injury,lowering blood lipids,and blood viscosity.Among them,the improvement of depressive symptoms is related to the serum ACTH and CORT contents.
