环池区的神经外科解剖学

Neurosurgical Anatomy of the Cisterna Ambiens

观察了35例(70侧)成人颅脑标本的环池。对环池的内容及周围主要结构间的相互关系作了观测。本文对至环池区的三种手术入路,即翼点入路、颞下入路和经侧脑室颞叶入路有关的定位标志作了探查。讨论了手术中处理桥静脉,保护滑车神经和小脑幕切迹与邻近的局部关系等问题。

Under an operative microscope,the cisterna ambiens was observed on 35 cases(70 sides)of the embalmed adult brains.The boundaries of the cistern were detailed.The topography of the structures within the cistern was outlined as well.Moreover,the principal brain structures near the cistern,such as the midbrain,pons,temporal lobe,inferior horn of the lateral ventricle and the Ⅱ to Ⅴ cranial nerves were dissected and measured.The anatomical landmarks in respect to the main surgical approaches,such as the pterional,the subtemporal and the temporal transventricular approaches,to the cisterna ambiens were suggested.The way to manage the bridge veins and protect the trochlear nerve during operation,and the relationship of the tentorial notch to the midbrain and the cerebellar tentorium were discussed.
