“互联网+”自我管理模式对结直肠癌患者社会心理适应和生活质量影响的meta分析

A Meta-analysis of the Effect of the “Internet+” Self-management Model on the Psychosocial Adaptation and Quality of Life on Colorectal Cancer Patients

目的:系统评价"互联网+"自我管理模式对结直肠癌患者自护能力、自我效能、社会心理适应和生活质量等的影响。方法:计算机检索Cochrane library、Web of Science、PubMed、中国知网、万方数据库、维普数据库搜集关于"互联网+"自我管理模式对结直肠癌患者干预效果的随机对照实验,检索时限均从建库至2020年10月。使用Cochrane5.1.0系统评价手册进行偏倚风险质量评价,对符合质量标准的文献采用RevMan5.3软件进行数据分析。结果:共纳入14篇文献,11篇中文文献,3篇英文文献,1492例结直肠癌患者。Meta分析显示:"互联网+"自我管理模式可以提高结直肠癌患者的自护能力[SMD=1.55,95%CI(1.32,1.77)]、自我效能[SMD=0.67,95%CI(0.43,0.90)]、生活质量[SMD=1.28,95%CI(0.40,2.15)],但对心理社会适应程度无明显改善[SMD=0.3,95%CI(-1.64,2.25)]。结论:"互联网+"自我管理模式能提高结直肠癌患者的自护能力、自我效能,改善患者整体生活质量,但对结直肠癌患者心理社会适应程度的影响仍需进一步验证。

Objective:To systematically evaluate the effect of the"Internet+"self-management model on self-care,self-efficacy,social psychological adaptation and quality of life in colorectal cancer patients.Methods:The randomized controlled trials regarding the effects of"Internet+"self-management model on colorectal cancer patients were collected through the databases,such as the Cochrane Library,Web of Science,PubMed,CNKI,Wanfang and VIP Database.The search time limit was from the construction of the library to October 2020.The Cochrane 5.1.0 System Evaluation Manual was used to evaluate the quality of the risk of bias,and the RevMan 5.3 software was used to analyze the data that met the quality standards.Results:Fourteen articles were included,including 3 English articles and 11 Chinese articles,with a total of 1492 patients.Meta-analysis showed that the application of the"Internet+"self-managemen model significantly improved the self-care of colorectal cancer patients[SMD=1.55,95%CI(1.32,1.77)],the self-efficacy of colorectal cancer patients[SMD=0.67,95%CI(0.43,0.90)]and the quality of life[SMD=1.28,95%CI(0.40,2.15)].But there was no significant improvement in the degree of psychosocial adaptation[SMD=0.3,95%CI(-1.64,2.25)].Conclusion:"Internet+"self-management mode can improve the self-car,self-efficacy and improve the overall quality of life of colorectal cancer patients.However,the effects on psychosocial adaptation and quality of life on physiological,psychological and spiritual dimensions of colorectal cancer patients still need further studies to prove.
