江西省某三甲医院新冠肺炎疫情防控期间医院感染现状调查

Current status of nosocomial infection in a three-A tertiary hospital of Jiangxi Province during prevention and control of COVID-19

目的了解新型冠状病毒肺炎疫情不同时期的医院感染流行趋势,为疫情下医院感染管理提供依据。方法采用病例调查的方式,调查某三甲医院2019年11月16日-2020年7月19日四个时期(疫情未发生时期、疫情流行暴发时期、聚集性散发时期、常态化防控时期)的医院感染情况,分析医院感染变化趋势。结果2019年11月16日-2020年7月19日应查出院人次数69101人次,四个时期医院感染率分别为1.30%、0.90%、0.59%、0.58%。疫情不同时期医院感染的年龄构成以>31岁的人群为主,感染病原体前三位分别为鲍氏不动杆菌复合菌、肺炎克雷伯菌、大肠埃希菌。患者发生医院感染的侵入性操作主要是呼吸机使用、泌尿道插管、中心静脉插管。四个时期医院入院患者发生医院感染核酸检测率分别为0、0.96%、0.59%、0.58%。结论医院新型冠状病毒肺炎疫情不同时期的医院感染率低于全国平均水平,研究了新型冠状病毒肺炎疫情不同医院感染的年龄构成、高危感染部位,为今后疫情下医院感染管理提供借鉴参考。

OBJECTIVE To understand the trend of nosocomial infection in different period of noval coronavirus pneumonia,and to provide the basis for the management of nosocomial infection under the epidemic situation.METHODS Case study was adopted to investigate the situation of nosocomial infection in a grade III level A hospital from Nov 16,2019 to Jul 19,2020.And the trend of nosocomial infection was analyzed.RESULTS During 16 Nov 2019 and 19 Jul 2020,69101 case-times patients were identified.The hospital infection rates in the four periods of the epidemic were 1.30%,0.90%,0.59%and 0.58%,respectively.The age composition of patients infected during different stages of the epidemic was mainly>31 years old,and the top three infectious pathogens were Acinetobacter baumannii complex,Klebsiella pneumoniae and Escherichia coli.The invasive operation causing nosocomial infection in patients was mainly the use of ventilators,urinary tract intubation,central intravenous intubation.The nucleic acid detection rates of nosocomial infection in hospital admission patients during the four periods were 0,0.96%,0.59%,and 0.58%,respectively.CONCLUSION The nosocomial infection rates of the new coronavirus in the hospital was lower than the national average in different periods of COVID-19.The age composition and high-risk infection sites of infections can provide reference for hospital infection management in the future.
