低剂量氟替卡松气雾剂吸入联合细菌溶解产物防治小儿反复喘息性疾病的效果研究

摘要 目的观察低剂量氟替卡松气雾剂吸入联合细菌溶解产物防治小儿反复喘息性疾病的效果,分析联合用药的有效性及安全性。方法选择2019年7月-2021年6月本院收治的反复发作哮喘患儿90例作为试验观察对象,参照随机数字表法将患儿分为对照组和试验组,每组45例。对照组患儿采取低剂量氟替卡松气雾剂吸入治疗,试验组患儿在对照组患儿的治疗基础上联合细菌溶解产物胶囊进行治疗,观察两组患儿治疗前血清炎性因子水平、免疫球蛋白及嗜酸性粒细胞(EOS)水平改善情况、药物不良反应发生情况、疾病控制情况及预后情况。结果治疗3个月后,试验组IL-4、IL-5及IL-12测定值均显著低于对照组,IFN-γ测定值明显高于对照组(P<0.05);试验组IgA、IgG测定值均显著高于对照组,EOS测定值明显低于对照组(P<0.05);对照组不良反应发生率为4.44%,试验组不良反应发生率为6.67%,组间比较无差异(P>0.05);试验组临床控制总有效率,显著高于对照组(P<0.05);治疗后1年内,试验组发作再就医次数明显少于对照组,无症状时长明显长于对照组(P<0.05)。结论低剂量氟替卡松气雾剂吸入联合细菌溶解产物在小儿反复喘息性疾病中的应用,可显著改善患儿机体炎症反应及免疫功能,提升患儿疾病控制总有效率,并改善患儿预后,且具有较高的安全性。 Objective To observe effect of low-dose fluticasone aerosol inhalation combined with bacterial lysates in prevention and treatment of recurrent wheezing diseases of children,and analyze effect and safety of the combined medication.Methods The paper chose 90 children with recurrent asthma admitted to our hospital from July 2019 to June 2021 as experimental observation objects,and divided them into control group and experimental group with random number table method,with 45 cases in each group.Control group was treated with low-dose fluticasone aerosol inhalation,while experimental group with bacterial lysate capsules on the basis of control group.Improvement of serum inflammatory factor levels,immunoglobulin and eosinophil(EOS)levels,incidence of adverse drug reactions,disease control,and prognosis between two groups before treatment were observed.Results After 3 months of treatment,measured values of IL-4,IL-5,and IL-12 in experimental group were significantly lower than control group.IFN-γmeasured values were significantly higher than control group(P<0.05).Measured values of IgA and IgG in experimental group were significantly higher than control group,while measured values of EOS were significantly lower than control group(P<0.05).Incidence of adverse reactions in control group was 4.44%,that of experimental group was 6.67%,with no difference between the groups(P>0.05).Total effective rate of clinical control in experimental group was 93.33%,significantly higher than 77.78%in control group(P<0.05).Within 1 year after treatment,times of experienced seizures and sought medical attention in experimental group were significantly lower than control group,and asymptomatic duration was significantly longer than control group(P<0.05).Conclusion Low-dose fluticasone aerosol inhalation combined with bacterial lysis products can significantly improve inflammatory response and immune function,enhance overall effective rate of disease control,and improve prognosis of children with recurrent wheezing diseases,with high safety.
