感染性休克中应用氢化可的松治疗的效果分析 被引量:1

摘要 目的探讨氢化可的松治疗感染性休克的效果。方法选择2020年1月-2021年12月本院重症医学科收治的60例感染性休克患者,采取随机分组方式将其分为对照组和观察组,每组各30例。对照组采用去甲肾上腺素、多巴胺治疗,观察组在去甲肾上腺素、多巴胺基础上联用氢化可的松治疗,对比两组间的总有效率、生命体征指标、急性生理和慢性健康评分、血清炎性因子指标、生活质量评分、机械通气时间、住院时间、死亡率。结果观察组总有效率比对照组高(P<0.05)。治疗后,观察组的心率、APACHE-Ⅱ评分均明显比对照组低(P<0.05),观察组平均动脉压明显比对照组高(P<0.05)。治疗后,观察组血清CRP、PCT水平均明显比对照组低(P<0.05);观察组较对照组的生活质量评分明显更高(P<0.05);观察组的机械通气时间、住院时间明显比对照组短(P<0.05);观察组的死亡率比对照组低(P<0.05)。结论联合应用去甲肾上腺素、多巴胺与氢化可的松治疗感染性休克,可有效控制患者病情,促使其生命体征恢复,减轻炎症反应,改善其生活质量,缩短其治疗时间。 Objective To explore effect of hydrocortisone in treatment of septic shock.Methods The paper chose 60 patients with septic shock admitted to ICU Department of our hospital from January 2020 to December 2021,and divided them into control group and observation group randomly,with 30 cases in each group.Control group was treated with norepinephrine and dopamine,while observation group with hydrocortisone on the basis of norepinephrine and dopamine.Total effective rate,vital sign indicators,acute physiological and chronic health scores,serum inflammatory factor indicators,life quality scores,mechanical ventilation time,length of stay,and mortality rate were compared between two groups.Results Total effective rate of observation group was,which was higher than control group(P<0.05).After treatment,heart rate and APACHE-Ⅱscores of observation group were significantly lower than control group(P<0.05),the average arterial pressure of observation group was significantly higher than control group(P<0.05).After treatment,serum CRP and PCT levels in observation group were significantly lower than control group(P<0.05).Life quality scores of observation group were significantly higher than control group(P<0.05).Mechanical ventilation time and length of stay of observation group was significantly shorter than control group(P<0.05).Mortality rate of observation group was lower than control group(P<0.05).Conclusion Norepinephrine,dopamine,combined with hydrocortisone can effectively control condition of septic shock patients,promote recovery of vital signs,reduce inflammatory reactions,improve life quality and shorten treatment time of patients.
