Gartland Ⅱ/Ⅲ型小儿肱骨髁上骨折保守治疗失败患者行闭合复位内固定的意义评价

Evaluation of the Significance of Closed Reduction and Internal Fixation in Patients with Type Gartland Ⅱ/Ⅲ Supracondylar Humerus Fractures in Children Whose Conservative Treatment Failed

目的分析对肱骨髁上骨折Ⅱ/Ⅲ型患儿采取闭合复位内固定治疗的临床疗效。方法选取本院于2019年1月-2021年12月收治的肱骨髁上骨折GartlandⅡ/Ⅲ型患儿60例,随机均分为两组,观察组(闭合复位交叉克氏针内固定治疗)和对照组(切开复位交叉穿针内固定治疗),分析两组临床疗效。结果疗效对比可见观察组相对更高,肘关节携带角丢失及屈伸功能丢失、肘关节屈伸活动及ROM评分明显优于对照组,并发症发生率对比可见观察组明显更低(P<0.05)。结论在肱骨髁上骨折GartlandⅡ/Ⅲ型患儿的临床治疗中用闭合复位内固定治疗效果明显更好,并发症发生率更低,值得推广。

Objective To analyze the clinical effect of closed reduction and internal fixation in children with typeⅡ/Ⅲsupracondylar humeral fractures.Methods A total of 60 children with GartlandⅡ/Ⅲsupracondylar fractures of the humerus admitted to our hospital from January 2019 to December 2021 were selected and randomLy divided into two groups:the observation group(closed reduction and cross Kirschner wire internal fixation)and the control group(open reduction and cross needle internal fixation).The clinical efficacy of the two groups was analyzed.Results The clinical effective rate of the observation group was significantly higher than that of the control group,the loss of elbow joint angle and flexion and extension function,elbow flexion and extension activities and ROM score were significantly better than those of the control group,and the incidence of complications was significantly lower than that of the control group(P<0.05).Conclusion In the clinical treatment of children with Gartland typeⅡ/Ⅲsupracondylar fractures of the humerus,closed reduction and internal fixation has better clinical efficacy and lower complication rate,which is worthy of promotion.
