术后尿管拔除的对比研究

A Comparative Study of Postoperative Catheter Extraction

目的研究术后不同尿管拔除方案的临床应用效果,旨在减轻患者痛苦以及经济负担。方法选取从2018年10月-2020年10月于本院接受腹部手术治疗的100例留置尿管患者作为研究对象。按照随机原则等分作艾灸组及对照组,每组各50例。对照组术后第1d开始定时夹闭及开放尿管,拔管前夹闭尿管,待膀胱充盈时拔除尿管。艾灸组术后第1d开始定时夹闭及开放尿管,拔管前开放尿管,促使膀胱空虚时拔除尿管,拔管之后予以艾灸,待艾灸结束之后告知患者自行排尿。比较两组临床疗效,术后并发症发生情况,尿失禁持续时间、住院时间以及住院费用。结果艾灸组治疗总有效率为96.00%,高于对照组的84.00%(P<0.05)。艾灸组术后并发症总发生率为2.00%,低于对照组的14.00%(P<0.05)。艾灸组尿失禁持续时间、住院时间分别为(3.12±0.15)d、(7.35±1.34)d,均短于对照组的(3.87±0.20)d、(10.82±1.75)d,而住院费用为(5123.48±311.74)元,少于对照组的(5912.83±405.94)元(P<0.05)。结论艾灸应用于术后尿管拔除的效果显著,有利于缩短尿失禁持续时间、住院时间,减少住院费用,同时有助于术后并发症的预防。

Objective To study and analyze the clinical effect of different postoperative catheter removal schemes,in order to reduce the pain and economic burden of patients.Methods From October 2018 to October 2020,100 patients with indwelling catheter underwent abdominal surgery in hospital were selected as the research object.According to the principle of randomness,moxibustion group and control group were divided into 50 cases in each group.In the control group,the catheter was clipped and opened regularly on day 1 after surgery.The catheter was clipped before extubation and removed when the bladder was filled.In the moxibustion group,the urethral duct was clipped and opened regularly on day 1 after surgery.The urethral duct was opened before extubation to promote the removal of the urethral duct when the bladder was empty.Moxibustion was performed after extubation,and the patients were told to urinate by themselves after the end of moxibustion.Clinical efficacy,postoperative complications,duration of urinary incontinence,length of stay and hospitalization costs were compared between the two groups.Results The total effective rate of moxibustion group was 96.00%,higher than that of control group 84.00%(P<0.05).The total incidence of postoperative complications in moxibustion group was 2.00%,lower than 14.00%in control group(P<0.05).The duration of urinary incontinence and hospital stay in the moxibustion group were(3.12±0.15)d and(7.35±1.34)d,respectively,which were shorter than those in the control group(3.87±0.20)d and(10.82±1.75)d,and the hospital cost was(5123.48±311.74)yuan,which was less than the control group(5912.83±405.94)yuan(all P<0.05).Conclusion Moxibustion has a significant effect on postoperative catheter extraction,which is beneficial to shorten the duration of urinary incontinence,the length of hospital stay,and the cost of hospital stay,as well as the prevention of postoperative complications,which is worthy of clinical promotion.
