高血压基底核出血实施传统开颅手术与3D-Slicer软件辅助神经内镜治疗的效果对比

摘要 目的分析传统开颅手术和3D-Slicer软件辅助神经内镜治疗高血压基底核出血的效果。方法研究样本选取为廊坊市第四人民医院2018年7月-2020年8月收治的高血压基底核出血患者62例。利用整群随机化分组法将其等分为研究组、对比组,其中对比组31例患者实施传统开颅手术治疗,研究组31例患者开展3D-Slicer+神经内镜治疗。对比分析两组ADL及NIHSS评分、术中失血量、手术时间、术后血肿残余量、住院天数。结果两组治疗前ADL及NIHSS评分差异不显著(P>0.05);研究组患者治疗后NIHSS及、ADL评分均好于对比组(P<0.05);研究组患者术中失血量、手术时间、术后血肿残余量、住院天数均低于对比组(P<0.05);两组术后并发症情况对比可知,研究组的总并发症发生率低于对比组(P<0.05);两组意识障碍程度分析可知,研究组与对比组术前评分无显著差异(P>0.05),手术1周后、2周后、4周后障碍评分均高于对比组,有显著差异(P<0.05);两组临床治疗有效率分析发现,研究组高于对比组,有显著差异(P<0.05)。结论3D-Slicer软件辅助神经内镜治疗高血压基底核出血能够取得理想疗效,改善患者神经功能与日常生活能力,意识障碍程度得到缓解,临床安全性提升,缩短患者治疗时间,推广应用价值较高。 Objective To analyze effect of traditional craniotomy and 3D slicer software assisted neuroendoscopy in treatment of hypertensive basal ganglia hemorrhage.Methods The paper chose 62 patients with hypertensive basal ganglia hemorrhage treated in Langfang City the Fourth People’s Hospital from July 2018 to August 2020 as study samples,and divided them into study group and control group by cluster randomization equally.31 cases in control group were treated with traditional craniotomy,and 31 cases in study group with 3D slicer+neuroendoscope.ADL and NIHSS scores,intraoperative blood loss,operation time,postoperative hematoma residual volume and hospital stay were compared and analyzed between two groups.Results There was no significant difference in ADL and NIHSS scores between two groups before treatment,(P>0.05);NIHSS and ADL scores in study group were better than control group(P<0.05);blood loss during operation,operation time,residual hematoma after operation and hospital stay in study group was lower than control group(P<0.05).Comparison of postoperative complications between two groups showed that total complications incidence in study group was lower than control group(P<0.05).According to analysis of consciousness disorders degree between two groups,there was no significant difference in preoperative scores between study group and control group,(P>0.05),obstacle scores after 1 week,2 weeks and 4 weeks of operation were higher than control group(P<0.05).Analysis of clinical treatment efficiency of study group was higher than control group,with significant difference between two groups,(P<0.05).Conclusion 3D slicer software assisted neuroendoscopy can achieve ideal curative effect in treatment of hypertensive basal ganglia hemorrhage,improve neural function and daily living ability of patients,alleviate consciousness disorder degree,improve clinical safety,shorten treatment time,which has high value of promotion and application.
