芬吗通应用于稽留流产患者清宫术后的效果分析

目的传统治疗下,患者清宫术后常因宫口松弛、抵抗力弱、创口感染和内膜不完全再生修复而致多种术后并发症发生。而芬吗通作为雌、孕激素的复合制剂,可以促进内膜增生与恢复。为此,兴宁市妇幼保健计划生育服务中心采用芬吗通应用于稽留流产患者清宫术的术后治疗并观察其效果。方法选取2020年1月31日-2020年12月31日收入本院的59例稽留流产患者纳入研究,采用随机数字表法分为对照组(30例)和观察组(29例)。对照组予以常规治疗,观察组在对照组的基础上予以芬吗通治疗,对比治疗前后两组患者的阴道出血情况、子宫内膜厚度、并发症(子宫粘连、积液及宫内感染)的发生率和不良反应。结果治疗后,观察组的阴道出血情况相较对照组恢复更好,即术后7d内出血量更少、阴道持续出血时长更短、月经恢复更快,两组对比差异具有统计学意义(P<0.05);治疗后7d,对照组子宫内膜厚度为(2.78±0.63)mm,观察组为(3.09±0.71)mm,两组对比差异无统计学意义(t=-1.772,P=0.082>0.05);而治疗后28d,观察组的子宫内膜厚度以(5.94±1.13)mm厚于对照组的(3.56±1.02)mm,两组对比差异具有统计学意义(t=-8.483,P=0.001<0.05);并发症的发生率20.69%低于对照组的46.67%(P<0.05);两组的不良反应无统计学差异(P>0.05)。结论芬吗通用于稽留流产患者清宫术后的治疗可以促进子宫内膜的恢复,降低并发症的发生概率。

Objective Under traditional treatment,patients often have a variety of postoperative complications due to uterine mouth relaxation,weak resistance,wound infection and incomplete regeneration and repair of intima.Fenbutone,as a compound preparation of estrogen and progesterone,can promote intimal hyperplasia and recovery.Therefore,fenbutone was applied to the postoperative treatment of uterine curettage in patients with missed abortion in our hospital,and its effect was observed.Methods 59 patients with missed abortion admitted to our hospital from January 31 to December 31,2020 were randomLy divided into control group(30 cases)and observation group(29 cases).The control group was treated with routine treatment,and the observation group was treated with fenbutone on the basis of the control group.The vaginal bleeding,endometrial thickness,incidence of complications(uterine adhesion,effusion and intrauterine infection)and adverse reactions were compared between the two groups before and after treatment.Results After treatment,the vaginal bleeding in the observation group recovered better than that in the control group,that is,less bleeding within 7 days after operation,shorter duration of vaginal bleeding and faster menstrual recovery(P<0.05);7 days after treatment,the endometrial thickness in the control group was(2.78±0.63)mm and that in the observation group was(3.09±0.71)mm,t=-1.772,P=0.082>0.05.There was no significant difference between the two groups;28 days after treatment,the thickness of endometrium in the observation group was(5.94±1.13)mm thicker than that in the control group(3.56±1.02)mm,t=-8.483,P=0.000<0.05);The incidence of complications was 20.69%lower than 46.67%in the control group(P<0.05);there was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Fentanyl can promote the recovery of endometrium and reduce the incidence of complications in patients with missed abortion.
