不同质子泵抑制剂联合奥曲肽治疗非静脉曲张性上消化道出血的成本、效果研究

Cost Effectiveness Study of Different Proton Pump Inhibitors Combined with Octreotide in the Treatment of Non Variceal Upper Gastrointestinal Bleeding

目的研究三种不同质子泵抑制剂联合奥曲肽治疗非静脉曲张性上消化道出血(NVUGIB)的成本-效果。方法前瞻性选取2020年1月-2021年3月惠东县人民医院收治的120例NVUGIB患者作为研究对象,根据随机原则分为3组,每组各40例。所有患者均予以奥曲肽治疗,E组联合艾司奥美拉唑治疗,O组联合奥美拉唑治疗,L组联合兰索拉唑治疗。对三组患者的临床疗效、止血时间、显效时间以及成本效果比(C/E)进行比较。结果三组临床总有效率相比,差异无统计学意义(P>0.05);三组止血时间、显效时间相比,E组优于O组、L组,差异有统计学意义(P<0.05);成本效果比(C/E)分析显示,E组优于O组、L组,差异有统计学意义(P<0.05)。结论艾司奥美拉唑、奥美拉唑、兰索拉唑分别联合奥曲肽治疗NVUGIB的效果相当,但艾司奥美拉唑能够快速止血,且从C/E来说具有更高的药物经济学价值。

Objective To study the cost-effectiveness of three different proton pump inhibitors combined with octreotide in the treatment of non variceal upper gastrointestinal bleeding(NVUGIB).Methods 120 patients with NVUGIB treated in Huidong County People’s Hospital from January 2020 to March 2021 were prospectively selected as the research object.They were randomly divided into three groups with 40 patients in each group.All patients were treated with octreotide,group E combined with esmoprazole,group O combined with omeprazole and group L combined with lansoprazole.The clinical efficacy,hemostasis time,effective time and C/E of the three groups were compared.Results There was no significant difference in the total clinical effective rate among the three groups(P>0.05);The hemostatic time and effective time of the three groups were statistically significant,and group E was better than group O and group L(P<0.05);Cost effectiveness ratio(C/E)analysis showed that group E was better than group O and group L(P<0.05).Conclusion Esomeprazole,Omeprazole And Lansoprazole combined with octreotide have the same effect in the treatment of NVUGIB,but esomeprazole can stop bleeding quickly and has higher Pharmacoeconomic value in terms of C/E.
