多囊卵巢综合征患者应用二甲双胍联合地屈孕酮治疗对血糖代谢及性激素水平的影响

Effect of Metformin Combined with Dydrogesterone on Blood Glucose Metabolism and Sex Hormone Levels in Patients with Polycystic Ovary Syndrome

摘要 目的研究多囊卵巢综合征患者应用二甲双胍联合地屈孕酮治疗对血糖代谢及性激素水平的影响。方法使用乱数表法将湖北省黄冈市中心医院收治的78例多囊卵巢综合征患者分为对照组(n=39)与治疗组(n=39),时间范围为2018年9月-2020年9月。对照组患者使用地屈孕酮,治疗组患者在此基础上联用盐酸二甲双胍片,两组治疗6个月经周期。比较两组患者治疗后临床疗效,治疗前后肥胖指标、血糖代谢指标及性激素水平。结果治疗组患者的临床总有效率为92.31%,高于对照组的71.79%;与治疗前比,治疗后两组患者血清LEP、VF、FINS、FPG、HOMA-IR、FSH、LH、E2水平及BMI均明显降低,且治疗组低于对照组(均P<0.05)。结论二甲双胍联合地屈孕酮治疗多囊卵巢综合征患者能有效改善肥胖状况,降低血糖及性激素水平,疗效显著。 Objective To investigate the effects of metformin combined with dydrogesterone on blood glucose metabolism and sex hormone levels in patients with polycystic ovary syndrome.Methods A total of 78 patients with PCOS admitted to our hospital were divided into control group(n=39)and treatment group(n=39)by random number table method from September 2018 to September 2020.Patients in the control group received dydrogesterone,and patients in the treatment group received metformin hydrochloride tablets in addition to dydrogesterone.The two groups were treated for 6 menstrual cycles.The clinical efficacy,obesity index,blood glucose metabolism index and sex hormone level before and after treatment were compared between the two groups.Results The total effective rate of the treatment group was 92.31%,higher than 71.79%of the control group.Compared with before treatment,serum LEP,VF,FINS,FPG,HOMA-IR,FSH,LH,E2 levels and BMI in 2 groups were significantly decreased after treatment,and those in treatment group were lower than those in control group(all P<0.05).Conclusion Metformin combined with dydrogesterone can effectively improve obesity,reduce blood glucose and sex hormone levels in patients with polycystic ovary syndrome.
