输尿管软镜钬激光碎石术治疗输尿管上段结石的疗效

Effect of Flexible Ureteroscope Holmium Laser Lithotripsy on Upper Ureteral Calculi

目的分析输尿管软镜钬激光碎石术治疗输尿管上段结石的临床疗效。方法将2019年10月-2020年12月徐闻县人民医院30例接受输尿管软镜钬激光碎石术治疗的输尿管上段结石患者设为观察组,另选同期的30例接受输尿管硬镜下钬激光碎石术治疗的输尿管上段结石患者设为对照组,对两组患者的平均手术时间、平均出血量、平均住院时间、一次结石清除率、不良反应发生情况进行对比分析。结果观察组患者的平均手术时间明显长于对照组,观察组患者的平均住院时间明显短于对照组,平均出血量明显低于对照组,差异具有统计学意义(P<0.05);观察组有28例患者结石一次清除,一次清除成功率为93.33%,对照组有21例患者结石一次清除,一次清除成功率为70.0%,观察组一次清除成功率明显高于对照组(P<0.05);观察组出现1例输尿管损伤,1例血尿,无感染发生,不良反应发生率为6.67%,对照组出现7例输尿管损伤,3例血尿,1例感染发生,不良反应发生率为36.67%,观察组不良反应发生率明显低于对照组(P<0.05)。结论输尿管软镜钬激光碎石术治疗输尿管上段结石的效果优于输尿管硬镜下钬激光碎石术。

Objective To analyze the clinical effect of flexible ureteroscope holmium laser lithotripsy in the treatment of upper ureteral calculi.Methods 30 patients with upper ureteral calculi underwent flexible ureteroscopic holmium laser lithotripsy From October 2019 to December 2020 in our hospital were selected as the observation group,and another 30 patients with upper ureteral calculi treated by rigid ureteroscope holmium laser lithotripsy were selected as the control group.The stone clearance rate and adverse reactions were compared and analyzed.Results The average operation time of the observation group was significantly longer than that of the control group,the average hospitalization time of the observation group was significantly shorter than that of the control group,the average blood loss was significantly lower than that of the control group,there was statistical significance(P<0.05).There were 28 patients in the observation group with one-time removal of stones,and the one-time removal success rate was 93.33%;there were 21 patients in the control group with one-time removal of stones,and the one-time removal success rate was 70.0%,the one-time removal success rate of the observation group was significantly higher than that of the control group(P<0.05);The observation group had 1 case of ureteral injury,1 case of hematuria,no infection,the incidence of adverse reactions was 6.67%,the control group had 7 cases of ureteral injury,3 cases of hematuria,1 case of infection,the incidence of adverse reactions was 36.67%,the incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The effect of flexible ureteroscope holmium laser lithotripsy in the treatment of upper ureteral calculi is better than that of rigid ureteroscope holmium laser lithotripsy.
