剖宫产术中不缝合皮下脂肪的双层缝合对子宫憩室形成率的影响

Effect of Double Layer Suture Without Suturing Subcutaneous Fat on the Formation Rate of Uterine Diverticulum in Cesarean Section

目的分析剖宫产术中不缝合皮下脂肪的双层缝合对子宫憩室形成率的影响。方法选择2020年6月-2021年6月广东省郁南县人民医院收治的100例行剖宫产术孕妇进行研究,根据切口缝合方式不同进行分组,对照组(n=49)行不缝合皮下脂肪单层缝合,观察组(n=51)行不缝合皮下脂肪双层缝合,分析疗效。结果两组手术时间(min)、术中出血量(mL)、术后恶露持续时间(d)、术后首次排气(d)、住院时间(d)等5项治疗指标比较,差异不显著(P>0.05)。观察组手术1年后憩室形成率低于对照组,差异有统计学意义(P<0.05),憩室容积小于对照组,差异有统计学意义(P<0.05),憩室厚度大于对照组,差异有统计学意义(P<0.05)。观察组皮下硬结、脂肪液化、切口红肿、切口愈合不良等不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论剖宫产术中不缝合皮下脂肪采用双层缝合,与单层缝合相比,手术用时、术中出血量等治疗指标无显著差异,但能明显降低子宫憩室形成率,且拆线时,不良反应发生率更低。

Objective To analyze the effect of double suture without suture of subcutaneous fat on the formation rate of uterine diverticulum during cesarean section.Methods 100 pregnant women undergoing cesarean section were divided into groups.The control group(n=49)received single-layer suture without subcutaneous fat,and the observation group(n=51)received double-layer suture without subcutaneous fat.The curative effect was analyzed.Results There was no difference between the two groups of five treatment indicators including operation time(min),intraoperative blood loss(mL),postoperative lochia duration(d),first postoperative exhaust(d),and hospital stay(d).Significant(P>0.05).The formation rate of diverticulum in the observation group was lower than that in the control group one year after the operation.The difference was statistically significant(P<0.05).The volume of the diverticulum was smaller than that of the control group.The difference was statistically significant(P<0.05).The thickness of the diverticulum was larger than that in the control group.Statistically significant(P<0.05).The total incidence of adverse reactions such as subcutaneous induration,fat liquefaction,incision swelling,and poor incision healing in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion In cesarean section,subcutaneous fat is not sutured and double suture is used.Compared with single-layer suture,there is no significant difference in treatment parameters such as operation time and intraoperative blood loss,but it can significantly reduce the formation rate of uterine diverticulum,and when the suture is removed,The incidence of adverse reactions is lower.
