摘要 目的分析四川省卫生人力资源配置公平性,为推动四川省卫生人力资源发展提供科学依据。方法采用卫生资源密度指数、泰尔指数和集聚度评价四川省卫生人力资源公平性。结果2019年四川省卫生技术人员、执业(助理)医师和注册护士卫生资源密度指数分别为2.99、1.10和1.34。四川省卫生技术人员、执业(助理)医师和注册护士按人口分布的泰尔指数分别为0.166、0.0180和0.0222,按地理分布的泰尔指数分别为0.5964、0.6464和0.6734。卫生技术人员、执业(助理)医师和注册护士中广元市、雅安市、阿坝州、甘孜州和凉山州的HRAD小于1,卫生技术人员中成都市、攀枝花市、雅安市和阿坝州HRAD/PAD大于1,执业(助理)医师中成都市、攀枝花市、德阳市、绵阳市和雅安市HRAD/PAD大于1,注册护士中成都市、攀枝花市、泸州市和雅安市HRAD/PAD大于1。结论卫生人力资源配置医护比不足,要加大对护理人员的培养力度。不公平性主要来自于区域内,要推动区域内卫生人力资源合理流动。存在卫生资源公平性与实际就医需求相悖的情况,要提高卫生人力资源配置的水平。 Objective Analyze the fairness of the allocation of health human resources in Sichuan Province and provide a scientific basis for promoting the development of health human resources in Sichuan Province.Methods The health resource density index,Theil index and agglomeration degree were used to evaluate the equity of health human resources in Sichuan Province.Results In 2019,the health resource density indexes of health technicians,practicing(assistant)physicians and registered nurses in Sichuan Province were 2.99,1.10,and 1.34,respectively.The Theil indices of health technicians,practicing(assistant)physicians and registered nurses in Sichuan Province are 0.166,0.0180 and 0.0222 respectively by population,and the Theil indices by geographical distribution are 0.5964,0.6464 and 0.6734 respectively.Health technicians,practicing(assistant)physicians and registered nurses have HRAD less than 1 in Guangyuan City,Ya’an City,Aba Prefecture,Ganzi Prefecture,and Liangshan Prefecture.Health technicians are HRAD/PAD in Chengdu,Panzhihua City,Ya’an City and Aba Prefecture Greater than 1,certified(assistant)physicians in Chengdu,Panzhihua,Deyang,Mianyang,and Ya’an have HRAD/PAD greater than 1,and registered nurses in Chengdu,Panzhihua,Luzhou,and Ya’an have HRAD/PAD greater than 1.Conclusion The allocation of health human resources is inadequate,and the training of nursing staff should be increased.Unfairness mainly comes from within the region,and it is necessary to promote the rational flow of health human resources within the region.There is a situation where the fairness of health resources is inconsistent with actual medical needs,and the level of health human resource allocation must be improved.
