腹腔镜下Soave根治术对小儿先天性巨结肠康复进程及疼痛程度的影响

Effects of Soave Radical Surgery under Laparoscopy on the Rehabilitation Process and Pain Degree of Children with Hirschsprung’s Disease

目的探讨在小儿先天性巨结肠患者治疗中腹腔镜下Soave根治术的应用对康复进程及疼痛程度的影响。方法选2014年9月-2020年9月在江门市中心医院治疗的60例小儿先天性巨结肠患者作为研究对象,均采用腹腔镜下Soave根治术治疗,对临床指标、治疗前后疼痛程度及功能恢复情况进行比较。结果60例小儿先天性巨结肠患儿均顺利完成腹腔镜下Soave根治术,平均手术时间为(89.32±7.48)min,术中平均出血量为(27.41±6.14)mL,术后平均排气时间为(2.31±0.43)d,术后平均进食时间为(2.35±0.35)d,平均住院时间为(7.28±1.05)d;术后1d,患儿视觉模拟评分法(VAS)评分较治疗前明显提高(P<0.05),术后3d VAS评分较术后1d明显下降(P<0.05);患儿Krickenbeck评分、Heikkinen评分术后7d、3个月较术前明显提高(P<0.05),术后3个月比术后7d明显提高(P<0.05)。结论在小儿先天性巨结肠患者治疗中腹腔镜下Soave根治术的使用能够降低疼痛程度,有利于康复治疗,效果显著。

Objective To explore the effect of laparoscopic Soave radical resection in the treatment of children with Hirschsprung’s disease on the rehabilitation process and the degree of pain.Methods A total of 60 cases of pediatric patients with congenital megacolon who received treatment in our hospital from September 2014 to September 2020 were selected as the research objects.All patients were treated with laparoscopic Soave radical resection.The clinical indicators,pain degree and functional recovery before and after treatment were compared.Results All the 60 children with congenital megacolon successfully completed laparoscopic Soave radical surgery.The mean operation time was(89.32±7.48)min,the mean intraoperative blood loss was(27.41±6.14)mL,the mean postoperative exhaust time was(2.31±0.43)d,the mean postoperative feeding time was(2.35±0.35)d,and the mean hospitalization time was(7.28±1.05)d.Visual analogue scale(VAS)scores were significantly increased after 1 day of operation compared with those before treatment(P<0.05),and decreased significantly after 3 days of operation compared with those after 1 day of operation(P<0.05).Krickenbeck score and Heikkinen score were significantly higher after 7 days and 3 months of operation than before operation(P<0.05),three months after operation,the score was significantly higher than that of the 7 day after operation(P<0.05).Conclusion In the treatment of children with Hirschsprung’s disease,the use of laparoscopic Soave radical resection can reduce the degree of pain,which is conducive to rehabilitation treatment,and the effect is significant.
