会阴无保护联合无痛分娩对初产妇自然分娩的过程及母婴结局的影响

Effect of Unprotected Perineum combined with Painless Delivery on Natural Delivery Process and Maternal and Infant Outcomes of Primiparas

目的探究初产妇应用会阴无保护联合无痛分娩对自然分娩过程及母婴结局的影响。方法样本选自我院妇产科2017年2月-2020年9月收治的100例产妇,随机分对照组和观察组,n=50。对照组产妇常规分娩,观察组则采用会阴无保护联合无痛分娩方式。对所有产妇的分娩指标、会阴裂伤程度、应激水平及新生儿情况进行比较。结果观察组较对照组第二产程时间、产后住院时间短,产后2h出血量少(P<0.05)。观察组产妇分娩后的会阴损伤程度显著低于对照组(P<0.05)。分娩后,两组产妇SOD及皮质醇水平均升高,且对照组组升高较观察组明显(P<0.05)。观察组较对照组新生儿窒息率低,1分钟和5分钟Apgar评分高(P<0.05)。结论初产妇应用会阴无保护联合无痛分娩可有效促进自然分娩过程,避免会阴的损伤,对产妇过度应激反应有效抑制,且母婴结局良好。

Objective To explore the effects of primiparas using perineum unprotected combined with painless delivery on the natural delivery process and maternal and infant outcome.Methods The samples were selected from 100 parturients admitted to the Department of Obstetrics and Gynecology in our hospital from February 2017 to September 2020,and were randomly divided into control and observation groups,n=50.The control group gave birth regularly,and the observation group used perineum unprotected combined painless delivery.Compare the delivery indicators,the degree of perineal laceration,the level of stress and the condition of the newborn for all mothers.Results Compared with the control group,the observation group had shorter time to the second stage of labor,shorter postpartum hospitalization,and less bleeding in the 2h postpartum(P<0.05).The degree of perineum injury in the observation group after childbirth was significantly lower than that in the control group(P<0.05).After delivery,the levels of SOD and cortisol in the two groups of parturients increased,and the increase in the control group was more significant than that in the observation group(P<0.05).Compared with the control group,the observation group had a lower neonatal asphyxia rate,and had higher Apgar scores at 1 minute and 5 minutes(P<0.05).Conclusion The use of perineum unprotected combined with painless delivery for primipara can effectively promote the natural delivery process,avoid perineal damage,effectively inhibit the excessive stress response of the puerpera,and have a good maternal and infant outcome.
