AMH、FSH、LH、E_(2)联合检测在不孕症患者卵巢储备功能中的预测价值

Predictive Value of AMH,FSH,LH and E_(2) combined Examination on the Ovarian Reserve Function of Infertility Patients

目的分析不孕症病患经抗缪勒管激素(AMH)、雌二醇(E_(2))、促卵泡激素(FSH)、黄体生成素(LH)检测对预测其卵巢储备功能的作用。方法将2020年1-12月本院收入的不孕症病患共68例为研究对象,均于入院后开展经阴道彩超检查,结合检查结果评定卵巢的储备功能情况,将窦卵泡数在5-15个者纳入到卵巢功能正常组(30例),将卵泡数<5个者纳入到卵巢功能降低组(38例),对比两组AMH、E_(2)、FSH及LH水平,同时观察单项和联合检测的阳性检出率情况。结果正常组病患的AMH、LH水平均高出下降组,E_(2)及FSH水平均低于下降组(P<0.05)。正常组病患的AMH、E_(2)、FSH及LH阳性检出率是6.67%、16.67%、6.67%、10.00%比下降组63.16%、73.68%、71.05%、65.79%更低(P<0.05)。下降组病患中联合AMH、E_(2)、FSH及LH几项指标的阳性检出率为92.11%比单项检测63.16%、73.68%、71.05%、65.79%更高(P<0.05)。结论不孕症病患的卵巢储备功能会受AMH、E_(2)、FSH及LH水平影响,结合几项指标水平对其卵巢储备功能具有良好预测作用,能为临床诊断和治疗提供重要参考依据。

Objective To analyze the predictive value of AMH(anti-mullerian hormone),E_(2)(estradiol),follicle-stimulating hormone(FSH)and luteinizing hormone(LH)combined examination on the ovarian reserve function of infertility patients.Methods 68 infertility patients treated from January 2020 to December 2020 in our hospital were selected as research objects.The transvaginal color Doppler sonography was performed to evaluate the ovarian reserve function.Patients with 5-15 follicles were included in normal ovarian reserve group(30 cases);patients with less than 5 follicles were included in diminished ovarian reserve group(38 cases).AMH,E_(2),FSH and LH levels were recorded;the positive detection rate based on single examination and combined examination was compared.Results AMH and LH levels in normal ovarian reserve group were higher than diminished ovarian reserve group;E_(2)and FSH levels were lower than diminished ovarian reserve group(P<0.05);positive detection rate of AMH,E_(2),FSH and LH in normal ovarian reserve group(6.67%,16.67%,6.67%,10.00%)was lower than diminished ovarian reserve group(63.16%,73.68%,71.05%,65.79%)(P<0.05);among diminished ovarian reserve group,the positive detection rate of AMH,E_(2),FSH and LH combined examination(92.11%)was higher than AMH,E_(2),FSH or LH examination(63.16%,73.68%,71.05%,65.79%)(P<0.05).Conclusion The infertility patient’s ovarian reserve function may be affected by AMH,E_(2),FSH and LH levels;the combined examination has good predictive value on the ovarian reserve function,providing basis on the clinical diagnosis and treatment.
