比较单孔腹腔镜与多孔腹腔镜卵巢囊肿剔除术的临床疗效及对术后卵巢储备功能的影响

Comparison of the Clinical Effects of Single-port Laparoscopic and Multi-port Laparoscopic Ovarian Cyst Removal and its Influence on Postoperative Ovarian Reserve

目的观察单孔腹腔镜与多孔腹腔镜卵巢囊肿剔除术的临床疗效及对术后卵巢储备功能的影响。方法在我院2019年11月至2020年11月收治的采用腹腔镜卵巢囊肿剔除术治疗的患者中,选取出88例展开研究,通过抽签的方式将其分为对照组(n=44)和研究组(n=44),对照组以三孔腹腔镜剔除术作为手术方案开展治疗,研究组以单孔腹腔镜剔除术作为手术方案开展治疗,比较组间手术相关指标、并发症发生情况以及卵巢储备功能指标在术前及术后30d的变化。结果组间术中出血量比较,研究组明显较对照组少(P<0.05),组间手术时间比较差异无统计学意义(P>0.05),组间住院时间比较,研究组明显较对照组短(P<0.05)。组间术后并发症率比较,研究组明显较对照组低(P<0.05)。组间术前各项卵巢储备功能指标比较无统计学意义(P>0.05),术后两组患者E2指标明显下降(P<0.05),FSH指标及LH指标明显上升(P<0.05),但组间比较无统计学意义(P>0.05)。结论单孔腹腔镜卵巢囊肿剔除术具有良好临床疗效,和多孔腹腔镜卵巢囊肿剔除术比较,其可以有效减少术后并发症的发生,并且不会对患者的卵巢储备功能带来更加明显的影响。

Objective To observe the clinical efficacy of single-port laparoscopic and multi-port laparoscopic ovarian cyst removal and their influence on postoperative ovarian reserve.Methods Among the patients treated with laparoscopic ovarian cyst removal in our hospital from November 2019 to November 2020,88 cases were selected for the study,and they were divided into a control group(n=44)and a study group(n=44)by lottery,the control group used three-port laparoscopic removal as the surgical plan for treatment,and the research group used single-port laparoscopic removal as the surgical plan for treatment.The operation-related indicators,complications,ovaries and changes of reserve function indexes before and 30 days after operation were compared between the groups.Results In comparison of intraoperative blood loss between the groups,the study group was significantly less than the control group(P<0.05);there was no significant difference in the operation time between the groups(P>0.05),and the hospital stay between the groups was significantly shorter than the control group(P<0.05).Comparing the postoperative complication rate between the groups,the study group was significantly lower than the control group(P<0.05).There was no statistically significant comparison of ovarian reserve indexes between groups before operation(P>0.05).After operation,the E2 index of the two groups decreased significantly(P<0.05),and the FSH index and LH index increased significantly(P<0.05),but the comparison between groups was not statistically significant(P>0.05).Conclusion Single port laparoscopic ovarian cyst removal has a good clinical effect.Compared with porous laparoscopic ovarian cyst removal,it can effectively reduce the occurrence of postoperative complications.And it will not have a more obvious impact on the patient’s ovarian reserve function.
