全椎板切除脊柱内固定治疗老年退行性腰椎管狭窄症的效果及VAS评分研究

Effect and VAS Scores Research of Total Laminectomy and Spinal Internal Fixation on Elderly Degenerative Lumbar Spinal Stenosis

目的探讨椎板完全切除脊柱内固定治疗老年退行性腰椎管狭窄症的效果及痛觉应激程度。方法回顾性分析广东省梅州市五华县人民医院2018年1月-2019年12月的30例老年退行性腰椎管狭窄症患者,根据治疗模式的差异分为部分切除组以及完全切除组,各15例。部分切除组实施椎板部分切除脊柱内固定术治疗,完全切除组给予椎板完全切除脊柱内固定治疗。比较治疗前后腰腿功能评分、痛觉应激程度、日本骨科协会评估治疗分数和生活质量、干预成效。结果完全切除组治疗后腰腿功能评分、痛觉应激程度低于部分切除组,日本骨科协会评估治疗分数和生活质量均高于部分切除组,干预成效高于部分切除组(P<0.05)。结论老年退行性腰椎管狭窄症在治疗中,采用椎板完全切除脊柱内固定的方式,有利于减轻痛觉应激,促使患者腰椎、生活质量改善。

Objective To investigate effect of complete laminectomy and spinal internal fixation in treatment of elderly degenerative LSS and pain stress degree.Methods The paper analyzed and reviewed 30 elderly patients with degenerative LSS in our hospital from January 2018 to December 2019,and divided them into partial resection group and complete resection group according to difference of treatment mode,with 15 cases in each group.Partial laminectomy group was treated with partial laminectomy and spinal internal fixation,and complete laminectomy group with complete laminectomy and spinal internal fixation.Waist and leg function scores,pain stress degree,treatment scores and life quality evaluated by the Japanese Orthopaedic Association,intervention effect were compared before and after treatment.Results After treatment,waist and leg function scores and pain stress degree in complete resection group were lower than partial resection group.Treatment scores and life quality assessed by Japanese Orthopaedic Association were higher than partial resection group,with higher intervention effect than partial resection group(P<0.05).Conclusion Complete laminectomy and spinal internal fixation can reduce pain stress and improve lumbar spine and life quality in treatment of elderly degenerative LSS.
