尿激酶治疗结核性胸膜炎的效果及对患者血清相关因子及不良反应的影响

Effect of Urokinase in Treatment of Tuberculous Pleurisy and on Serum Related Factors and Adverse Reactions

目的分析尿激酶治疗结核性胸膜炎效果及其对血清相关因子及不良反应的影响。方法采用随机数字表法将86例结核性胸膜炎患者分为对照组和病例组,各43例,均给予抗结核治疗,病例组加用尿激酶治疗,对比疗效、引流总量、住院时间、血清相关因子、肺功能指标、不良反应。结果两组治疗前LDH、ADA、FEV1、FVC水平差异无统计学意义(P>0.05),病例组治疗后治疗总有效率高于对照组,且引流总量更多,住院时间更短,LDH、ADA水平更低,FEV1、FVC水平更高,差异有统计学意义(P<0.05)。两组肝功能异常、胸痛、发热、胸膜增厚、包裹性积液等不良反应总发生率差异无统计学意义(P>0.05)。结论结核性胸膜炎患者给予尿激酶疗效及安全性俱佳,可缓解炎症反应,促进肺功能恢复。

Objective To analyze effect of urokinase in treatment of tuberculous pleurisy and influence on serum related factors and adverse reactions.Methods The paper divided 86 cases tuberculous pleurisy patients into control group and case group with random digital table method,with 43 cases in each group.Both groups were treated with anti tuberculosis treatment,case group was added with urokinase.Curative effect,total drainage,length of stay,serum related factors,pulmonary function indexes and adverse reactions were compared between two groups.Results There was no significant difference in LDH,ADA,FEV1 and FVC levels between two groups before treatment(P>0.05).Total effective rate of case group after treatment was higher than control group,with more total drainage,shorter hospital stay,lower levels of LDH and ADA,and higher levels of FEV1 and FVC,difference was statistically significance,(P<0.05).There was no significant difference in total incidence of adverse reactions including abnormal liver function,chest pains,fever,pleural thickening and encapsulated effusion between two groups(P>0.05).Conclusion Urokinase is effective and safe for tuberculous pleurisy patients,can alleviate inflammatory reaction and promote recovery of pulmonary function,which is worthy of promotion.
