FTS用于剖宫产术后预防深静脉血栓的Meta分析

A Meta-analysis of Fast Track Surgery for Cesarean Delivery and Deep Venous Thrombosis

目的通过Meta分析定量评估FTS(fast track surgery)在剖宫产围术期应用对预防下肢静脉血栓的效果,进一步为推广FTS在产科应用提供理论依据。方法利用计算机检索文献,检索时间为从建库开始至2021年7月31日,收集以FTS为干预措施在剖宫产围术期应用的随机对照试验(random control test,RCT),采用Cochrane协作网系统评价方法筛选文献,用Stata 12.0软件对符合标准纳入的文献进行Meta分析。结果Meta分析一共纳入8篇文献,DVT发生有关8篇,下肢肿胀6篇,静脉曲张2篇,合并后RR值分别为0.16(95%CI:0.09-0.31;P=0.000),0.28(95%CI:0.20-0.40;P=0.000),0.17(95%CI:0.04-0.72;P=0.017)。D-D聚体相关2篇,合并后WMD为54.833(95%CI:-441.428,551.195;P=0.828)。结论剖宫产围术期应用FTS可以降低术后深静脉血栓形成的风险,但需要更多高质量的研究进一步分析。

Objective This study aims at conducting a Meta-analysis to synthetically quantify the effect of FTS(fast track surgery)in the perioperative period of cesarean delivery on the prevention of deep venous thrombosis,and to provide a theoretical basis for promoting the application of FTS in obstetrics.Methods The literatures were retrieved by computer,using the Cochrane Collaboration network system evaluation method to obtain all random control tests(RCT)which use FTS as an intervention in the perioperative period of cesarean delivery,using Stata 12.0 software to do the Meta-analysis on the literature comply with the standards.Results A total of 8 articles were included for Meta-analysis.There were 8 related to DVT,6 related to lower limb swelling,and 2 related to varicose veins.The combined RR values were 0.16(95%CI:0.09-0.31;P=0.000),0.28(95%CI:0.20-0.40;P=0.000),and 0.17(95%CI:0.04-0.72;P=0.017).Two articles are related to D-D aggregates,and combined WMD was 54.833(95%CI:-441.428,551.195;P=0.828).Conclusion Perioperative application of FTS for cesarean delivery can reduce the risk of deep venous thrombosis,but more high-quality studies are needed for further analysis.
