老年性睑外翻伴泪道阻塞的手术治疗效果

Surgical Treatment of Senile Ectropion with Lacrimal Duct Obstruction

目的观察老年性睑外翻合并泪道阻塞患者行人工泪管置入或泪囊鼻腔吻合联合外侧睑板固定-水平缩短术治疗效果。方法回顾性分析31例老年性下睑外翻合并泪道阻塞患者病例手术资料,根据泪道阻塞部位,术中采用的不同的术式,其中23例患者术中行人工泪管置入联合外侧睑板固定-水平缩短术,8例行泪囊鼻腔吻合术+人工泪管植入联合外侧睑板固定-水平缩短术,术后随访6个月。结果术后所有患者眼睑形态好,眼睑位置佳,无眼睑外翻,泪点位置好,冲洗泪道通畅,溢泪症状明显改善。结论对于老年性眼睑外翻合并泪道阻塞患者,应用泪道人工泪管植入或泪囊鼻腔吻合联合外侧睑板固定-水平缩短术是安全有效的。

Objective To observe the effect of artificial lacrimal duct catheterization or dacryocystorhinostomy combined with lateral eyelid fixation-horizontal shortening on patients with senile ectropion and obstruction of lacrimal passage.Methods There is a retrospective analysis to 31 cases of patient who undergoing senile ectropion and obstruction of lacrimal passage.According to the position of obstruction of lacrimal passage,patients ware treated in different ways.The 23 cases ware treated with artificial lacrimal duct catheterization and lateral eyelid fixation-horizontal shortening.The other 8 cases were treated with dacryocystorhinostomy combined with lateral eyelid fixation-horizontal shortening.After operation,we follow up 6 months.Results After operation,all patient’s eyelid had acquired a good appearance,suitable position,without ectropion and their dacryon had suitable position and their lacrimal passage not be obstructed and their epiphora had improved obviously.Conclusion For senile ectropion with lacrimal duct obstruction,lacrimal duct implantation or dacryocystorhinostomy combined with lateral tarsal plate fixation horizontal shortening is safe and effective.
