脑干听觉诱发电位鉴别诊断新生儿胆红素脑病与高胆红素血症的效果分析

摘要 目的探讨脑干听觉诱发电位诊断新生儿胆红素脑病敏感度及特异度。方法回顾性分析2014年5月至2019年9月我院收治的89例新生儿胆红素脑病患儿的临床资料,全部患儿均接受脑干听觉诱发电位检查,分析脑干听觉诱发电位诊断新生儿胆红素脑病敏感度及特异度。结果胆红素脑病组与高胆红素血症组基线资料差异无统计学意义(P>0.05);胆红素脑病组黄疸出现时间与黄疸高峰时间短于高胆红素血症组,黄疸持续时间长于高胆红素血症组(P<0.05);胆红素脑病组UCB、TSB与B/A值高于胆红素血症组(P<0.05);胆红素脑病组的平均阈值高于高胆红素血症组(P<0.05);脑干听觉诱发电位异常诊断胆红素脑病的灵敏度、特异度与头颅CT检查差异无统计学意义(P>0.05)。结论脑干听觉诱发电位诊断新生儿胆红素脑病敏感度及特异度均较高,是诊断胆红素脑病的有效方法。 Objective To investigate the sensitivity and specificity of brainstem auditory evoked potentials in the diagnosis of neonatal bilirubin encephalopathy.Methods The clinical data of 89 neonates with bilirubin encephalopathy admitted to our hospital from May 2014 to September 2019 were retrospectively analyzed.All children underwent brainstem auditory evoked potential examination,and the sensitivity and specificity of the diagnosis of neonatal bilirubin encephalopathy were analyzed.Results There was no significant difference in baseline data between the bilirubin encephalopathy group and the hyperbilirubinemia group(P>0.05);the jaundice appearance time and peak time of jaundice in the bilirubin encephalopathy group were shorter than those in the hyperbilirubinemia group.The duration of jaundice was longer than that of the hyperbilirubinemia group(P<0.05);the UCB,TSB and B/A values of the bilirubin encephalopathy group were higher than those of the bilirubinemia group(P<0.05);The average threshold was higher than that in the hyperbilirubinemia group(P<0.05);the sensitivity and specificity of abnormal brainstem auditory evoked potentials in the diagnosis of bilirubin encephalopathy were not statistically different from those of the head CT examination(P>0.05).Conclusion Brainstem auditory evoked potentials have high sensitivity and specificity in the diagnosis of neonatal bilirubin encephalopathy,and it is an effective method for the diagnosis of bilirubin encephalopathy.
