观察胃癌根治术联合改良胃转流手术治疗胃癌合并2型糖尿病的临床疗效

Observation of the Clinical Effect of Radical Gastrectomy Combined with Modified Gastric Bypass Surgery in the Treatment of Gastric Cancer with Type 2 Diabetes Mellitus

目的观察胃癌合并2型糖尿病患者选择胃癌根治术联合改良胃转流手术治疗的效果。方法将60例湛江市南油医院于2018年9月至2020年9月救治的胃癌合并2型糖尿病患者作为研究对象,将其随机分两组,各30例,对照组采取胃癌根治术治疗,观察组联合改良胃转流手术,对比不同方案对患者的影响。结果治疗前,两组患者指标无较大差异,治疗后,观察组患者的血糖指标下降更明显,糖化血红蛋白、空腹C-肽指标更优,而且胃肠功能恢复时间、下床活动时间、进食流质食物时间较对照组短,观察组出现并发症例数少于对照组,差异有统计学意义(P<0.05),两组患者体重指数对比差异无统计学意义(P>0.05)。结论胃癌根治术联合改良胃转流手术在治疗胃癌合并2型糖尿病效果明显,改善患者的血糖等指标,加快术后康复速度,值得重视。

Objective To observe the effect of gastric cancer combined with modified gastric bypass surgery in patients with type 2 diabetes mellitus.Methods From September 2018 to September 2020,60 patients with gastric cancer complicated with type 2 diabetes in Nanyou Hospital in Zhanjiang were randomly divided into two groups,30 cases each,and the control group was treated with radical gastrectomy.The observation group combined with modified gastric bypass surgery compared the effects of different schemes on patients.Results There was no significant difference between the two groups before treatment.After treatment,the blood glucose index of the observation group decreased more obviously,glycosylated hemoglobin,fasting C-peptide index was better,and the recovery time of gastrointestinal function,the time of getting out of bed and eating fluid food were shorter than that of the control group.The number of complications in the observation group was less than that in the control group(P<0.05),and the difference of body mass index between the two groups was smaller(P<0.05).Conclusion The effect of radical resection of gastric cancer combined with modified gastric bypass surgery in the treatment of gastric cancer with type 2 diabetes is obvious,the blood sugar of patients is improved,and the speed of postoperative rehabilitation should be accelerated.
