运用Andrews测量法评估骨性Ⅱ类患者上下颌骨的相关研究

摘要 目的运用Andrews测量法评估骨性Ⅱ类患者上下颌骨并进行相关对比研究。方法选取恒牙牙合骨性Ⅱ类错牙合畸形患者头颅侧位片306例,对比Andrews测量法和SNA/SNB角两种评估方法,同时将Andrews测量法数据根据年龄段和性别分组,进行配对χ^(2)检验。结果使用Andrews测量法与SNA/SNB角两种不同评估方法,配对χ^(2)检验(P_(1)=0.0001<0.05,Kappa_(1)=0.02<0.4;P_(2)=0.0001<0.05,Kappa_(2)=0.037<0.4);不同年龄组间P_(a)=0.004<0.05、P_(b)=0.001<0.05;不同性别组间P_(a)=0.860>0.05、P_(b)=0.274>0.05。结论Andrews测量法与SNA、SNB角比较具有显著差异且诊断结果一致性较差;Andrews测量法在不同年龄组间上、下颌骨矢状向发育均具有显著差异,与性别无关;Andrews测量方法可以作为一种新的评估骨性Ⅱ类患者上下颌骨矢状向发育的方法,供临床鉴别应用。 Objective The aim of this study was to evaluate of maxilla and mandible in patients with skeletal ClassⅡby Andrews measurement and carry out the comparative study.Methods 306 patients with skeletal ClassⅡmalocclusion were selected to compare the SNA/SNB angle and Andrews measurement.The samples were grouped according to age and gender,and McNemar’s test was performed.Results Using Andrews measurement and SNA/SNB angle,the following results are obtained:McNemar’s test(P_(1)=0.0001<0.05,Kappa_(1)=0.02<0.4;P_(2)=0.0001<0.05,Kappa_(2)=0.037<0.4);Between different age groups P_(a)=0.004<0.05,P_(b)=0.001<0.05;Between different gender groups P_(a)=0.860>0.05,P_(b)=0.274>0.05.Conclusion There were significant differences between Andrews and SNA/SNB,and the consistency of diagnosis results was poor.There were significant differences in sagittal direction of maxilla and mandible among different age groups,and the results were not gender related.Andrews measurement method can be used as a new method to evaluate the sagittal development of maxilla and mandible in patients with skeletal ClassⅡ.
