关节内外联合神经阻滞治疗骶髂关节痛的疗效观察

Observations of Intra-articular and Extra-articular Nerve Block in Treating Sacroiliac Joint Pain

目的探讨关节内外联合神经阻滞治疗骶髂关节痛的临床效果。方法研究纳入东莞市滨海湾中心医院收治的骶髂关节痛患者80例,研究开展时间在2018年10月至2020年1月。回顾性分析患者的临床资料,根据其神经阻滞治疗方式的不同将80例患者分为2组,其中参与关节内神经阻滞治疗的39例患者纳入对照组,其余41例采用关节内外联合神经阻滞治疗的患者纳入研究组,对比2组患者的临床治疗效果。结果治疗前,对照组和研究组疼痛评分对比差异无统计学意义(P>0.05);治疗结束时和治疗结束后2周,2组疼痛评分较治疗前相比均有明显下降,组内对比差异有统计学意义(P<0.05),并且研究组治疗结束时和治疗结束后2周的疼痛评分均低于对照组,组间差异有统计学意义(P<0.05)。2组治疗前腰椎功能障碍评分对比差异无统计学意义(P>0.05);治疗后结束后2周,2组腰椎功能障碍评分均低于本组治疗前,组内对比差异有统计学意义(P<0.05),研究组治疗结束后2周腰椎障碍功能评分明显低于对照组,组间对比差异有统计学意义(P<0.05)。治疗前,2组生理领域、心理领域、环境领域和社会关系领域四个维度生活质量评分对比差异无统计学意义(P>0.05);治疗结束后2周,2组上述四个维度评分均有明显提高,组内对比差异有统计学意义(P<0.05),研究组治疗结束后2周四个维度评分均高于对照组,组间对比差异有统计学意义(P<0.05)。结论关节内外联合神经阻滞可以有效减轻骶髂关节痛患者的疼痛症状,在改善患者腰椎功能障碍的同时提高其生活质量,临床治疗效果显著。

Objective To discuss the clinical effects of intra-articular and extra-articular nerve block in treating sacroiliac joint pain.Methods Clinical data about 80 sacroiliac joint pain patients treated from October 2018 to January 2020 in Binhaiwan Central Hospital of Dongguan City were reviewed.According to different nerve block modes,39 cases receiving the intra-articular nerve block were set as control group;41 cases receiving the intra-articular and extra-articular nerve block were set as research group.Clinical effects were probed.Results Before treatment,pain scores between groups were not significantly different(P>0.05);after treatment and after 2w of treatment,pain scores for two groups were remarkably declined(P<0.05);after treatment and after 2w of treatment,pain scores in the research group were even lower than control group(P<0.05);before treatment,Oswestry disability index between groups were not significantly different(P>0.05);after treatment and after 2w of treatment,Oswestry disability index for two groups were remarkably declined(P<0.05);after treatment and after 2w of treatment,Oswestry disability index in the research group were even lower than control group(P<0.05);before treatment,the assessment scores of physiological function,mental function,environmental factors and social relation between groups were not significantly different(P>0.05);after treatment and after 2w of treatment,life quality scores for two groups were remarkably increased(P<0.05);after treatment and after 2w of treatment,life quality scores in the research group were even higher than control group(P<0.05).Conclusion The intra-articular and extra-articular nerve block can relieve the pain degree and lumbar vertebral dysfunction,and improve the life quality and treatment effects.
