基于家庭医生签约服务的社区高血压管理效果评价

Effect Evaluation of Community Hypertension Management Based on Family Doctor Contract Service

目的探究基于家庭医生签约服务模式的社区高血压管理的效果。方法选取我中心150例高血压患者进行研究,分为两组,每组各75例。对照组按照国家基本公共卫生服务规范采取高血压慢病规范化管理,观察组在对照组管理的基础上采用家庭医生签约服务模式,开展为期1年的管理,观察两组患者在管理前后收缩压、舒张压及生活方式改变、依从性等变化情况并进行比较。结果两组患者在管理前年龄、病程、血压差异无统计学意义(P>0.05),具有可比性;管理后,收缩压、舒张压都比管理前降低,且实施家庭医生签约服务的观察组下降比对照组更为显著,比较存在统计学意义(P<0.05),在依从性和生活方式的改善上,管理后观察组优于对照组,差异有统计学意义(P<0.05)。结论基于家庭医生签约服务的社区高血压患者管理,可有效控制血压,改变不良生活方式,减少高危因素出现,可有效预防并发症的发生,值得临床推广。

Objective To explore the effect of community hypertension management based on the family doctor contract service model.Methods 150 cases of patients with hypertension were selected and divided into two groups,75 cases in each group.The control group adopted standardized management of hypertension and chronic diseases according to the national basic public health service standards,while the observation group adopted family doctor contract service mode on the basis of the control group management and received management for one year.The changes of systolic blood pressure,diastolic blood pressure,lifestyle and compliance of the two groups before and after management were observed.Results There was no significant difference in age,course of disease and blood pressure between the two groups before management(P>0.05),which was comparable;after management,systolic blood pressure and diastolic blood pressure were lower than before management,and the decrease in the observation group with family doctor signing service was more significant than that in the control group,which was statistically significant(P<0.05).The difference was statistically significant(P<0.05).Conclusion The management of hypertension patients in community based on family doctor contract service can effectively control blood pressure,change unhealthy lifestyle,reduce the occurrence of high-risk factors,and effectively prevent the occurrence of complications,which is worthy of clinical promotion.
