双水平无创呼吸机对肺心病合并睡眠呼吸暂停患者的疗效及血清内皮素-6、C反应蛋白及肿瘤坏死因子-α水平的影响

Effect of Bi Level Noninvasive Ventilator on Patients With Cor Pulmonale Complicated With Sleep Apnea and Levels of Serum Endothelin-6,C-Reactive Protein and Tumor Necrosis Factor-α

目的观察双水平无创呼吸机对肺源性心脏病(PHD)合并呼吸睡眠暂停(OSAHS)患者的疗效及对患者血清内皮素-6(ET-6)、C反应蛋白(CPR)、肿瘤坏死因子-α(TNF-α)的影响。方法选择60例2018年6月至2020年6月间在我院诊治的肺源性心脏病合并睡眠呼吸暂停患者为研究对象,分为治疗组(30例)和对照组(30例)。对照组给予内科常规治疗,治疗组在此基础上再予以双水平无创呼吸机治疗,比较对照组与治疗组患者治疗前后治疗效果、体征、Sa(O2)、血气分析情况及血清ET-6、CPR、TNF-α参数变化。结果治疗组患者临床症状、体征的改善及血气分析好转较对照组更迅速而且更显著;且治疗组患者ET-6、CPR、TNF-α水平下降明显(P<0.001)。结论双水平无创呼吸机治疗肺源性心脏病合并睡眠呼吸暂停患者取得良好疗效同时有效降低ET-6、CRP、TNF-α水平,减少机体的炎症反应,提高临床治疗效果。

Objective To observe effect of bi level noninvasive ventilator on patients with pulmonary heart disease(PhD)combined with sleep apnea(OSAHS)and serum endothelin-6(et-6),C-reactive protein(CPR)and tumor necrosis factor-α(TNF-α).Methods The paper chose 60 cases pulmonary heart disease patients complicated with sleep apnea in our hospital from June 2018 to June 2020 as research objects and divided them into treatment group(30 cases)and control group(30 cases).Control group was treated with routine medical treatment,and treatment group with double level noninvasive ventilator treatment on the basis.Treatment effect,physical signs,SA(O2),blood gas analysis,serum ET-6,CPR and TNF-αparameters were compared between control and treatment group before and after treatment.Results Improvement of clinical symptoms and signs and blood gas analysis in treatment group was faster and more significant than control group,levels of ET-6,CPR and TNF-αin treatment group decreased significantly(P<0.001).Conclusion Double level noninvasive ventilator has good curative effect in treatment of patients with pulmonary heart disease and sleep apnea,and reduce levels of ET-6,CRP and TNF-αeffectively,reduce inflammatory reaction of the body,and improve clinical treatment effect.
