Bakri球囊填塞与B-Lynch缝合术治疗剖宫产产后出血比较分析

A Comparative Analysis of Bakri Balloon Packing and B-Lynch Suture in the Treatment of Postpartum Hemorrhage after Cesarean Section

目的对Bakri球囊填塞与B-Lynch缝合治疗剖宫产产后出血实际效果进行比较。方法回顾性分析我院产科2019年7月至2020年7月间接收的94例剖宫产产后出血患者,根据治疗方案的不同分为A、B两组,每组各47例。A组以Bakri球囊填塞进行治疗,B组采取B-Lynch缝合术治疗,对比两组患者2h出血量、术后24h出血量、输血量、止血有效率以及术后并发症发生率。结果A组患者术后2h出血量要低于B组(t=5.241,P=0.001);A组术后24h出血量要低于B组(t=18.669,P=0.001);A组输血量要低于B组(t=7.651,P=0.001)。A组止血有效率(95.74%)与B组(91.49%)比较,组间差异无统计学意义(χ^(2)=0.712,P=0.399)。A组并发症发生率(2.13%)较B组(14.89%)更低,组间差异有统计学意义(χ^(2)=4.919,P=0.027)。结论Bakri球囊填塞与B-Lynch缝合术治疗剖宫产产后出血均能够有效止血,但Bakri球囊填塞具有一定优势,术后出血量更低,输血量更少,并发症更少。

Objective To compare the actual effects of Bakri balloon packing and B-Lynch suture in the treatment of post-cesarean section hemorrhage.Methods 94 cases of postpartum hemorrhage after cesarean section in obstetrics department of our hospital from July 2019 to July 2020 were retrospectively analyzed,and they were divided into two groups according to different treatment schemes,47 cases in each group.Group A was treated with Bakri balloon tamponade and group B was treated with B-Lynch suture.The amount of bleeding in 2 hours,24 hours after operation,blood transfusion,hemostasis efficiency and incidence of postoperative complications were compared between the two groups.Results The amount of bleeding in group A was lower than that in group B(t=5.241,P=0.001),the amount of bleeding in group A was lower than that in group B(t=18.669,P=0.001),the amount of blood transfusion in group A was lower than that in group B(t=7.651,P=0.001).There was no significant difference between group A(95.74%)and group B(91.49%)(χ^(2)=0.712,P=0.399).The incidence of complications in group A(2.13%)was lower than that in group B(14.89%)(χ^(2)=4.919,P=0.027).Conclusion Bakri balloon tamponade and B-Lynch suture can effectively stop bleeding after cesarean section.However,Bakri balloon tamponade has certain advantages,including lower postoperative blood loss,less blood transfusion,and fewer complications.
