结直肠息肉内镜下黏膜切除术(EMR)与内镜下氩离子血浆凝固术(APC)的治疗效果对比及分析

Comparison and Analysis of Therapeutic Effect between Endoscopic Mucosal Resection(EMR)of Colorectal Polyps and Endoscopic Argon Ion Plasma Coagulation(APC)

目的研究对结直肠息肉患者分别采用内镜下黏膜切除术(EMR)和内镜下氩离子血浆凝固术(APC)的临床效果。方法选择2018年5月至2021年5月至我院消化科进行结直肠息肉治疗的患者60例,按其治疗方式分为两组,每组30例,分别为内镜下黏膜切除术治疗的EMR组和内镜下氩离子血浆凝固术治疗的APC组,分别观察两组患者的围手术期指标、治疗效果、术后并发症的发生情况及息肉复发率。结果两组患者经治疗后效果均较为显著,无明显差异(P>0.05),但EMR组总有效率(100.00%)略高于APC组(96.67%);EMR组手术时间比APC组患者长(P<0.05),住院时间相似,EMR组术后并发症发生率(10.00%)比APC组(6.67%)略高(P>0.05),其息肉复发率(3.33%)比APC组(13.33%)低(P<0.05)。结论EMR和APC疗效皆较为显著,但相比之下,采用EMR进行治疗患者术后不易出现严重并发症,且结直肠息肉的复发率也更低,建议在临床加以推广。

Objective To investigate the clinical effects of endoscopic mucosal resection(EMR)and argon ion plasma coagulation under endoscope(APC)in patients with colorectal polyps.Methods Select in May 2018 to May 2021 to our digestive department for 60 patients with colorectal polyps treatment,according to the treatment methods are divided into two groups,each group 30 cases,respectively,they are the EMR group treated by endoscopic mucosal resection and the APC group treated by endoscopic argon plasma coagulation,respectively to observe perioperative indexes of two groups of patients,the treatment effect,the occurrence of postoperative complications and polyp recurrence rate.Results After treatment,there was no significant difference between the two groups(P>0.05),but the EMR group(100.00%)was slightly higher than the APC group(96.67%).The operative time of EMR group was longer than that of APC group(P<0.05),the length of hospital stay was similar,the incidence of postoperative complications in EMR group(10.00%)was slightly higher than that of APC group(6.67%)(P>0.05),and the polyp recurrence rate(3.33%)was lower than that of APC group(13.33%)(P<0.05).Conclusion Both EMR and APC have significant efficacy,but in comparison,patients treated with EMR are less likely to have serious postoperative complications,and the recurrence rate of colorectal polyps is lower,so it is recommended to promote them in clinical practice.
