异位妊娠通过阴道超声和腹部超声检测技术的应用意义

Clinical Significance of Transvaginal Ultrasound and Transabdominal Ultrasound in Ectopic Gestation

目的分析经阴道超声以及腹部超声对异位妊娠的诊断价值。方法对2019年1月至2019年12月本院经手术和病理确诊是异位妊娠的78例患者的临床资料开展回顾性分析,均先行腹部超声检查,后行阴道超声检查,统计并比较两种检查方式的诊断准确率、超声图像表现、显示孕囊时间、确诊时间、子宫内膜厚度与子宫内膜血流RI的检出情况。结果阴道超声的诊断准确率为97.44%比腹部超声82.05%更高(P<0.05)。阴道超声对盆腔积液、假孕囊、附件包块、心血管搏动以及胚芽的检出率分别为92.31%、19.23%、97.44%、17.95%、20.51%比腹部超声64.10%、3.85%、82.05%、3.85%以及5.13%更高(P<0.05)。阴道超声的显示孕囊时间和确诊时间分别为(42.85±5.96)d、(45.48±6.75)d比腹部超声(52.50±6.18)d、(56.35±7.24)d更短(P<0.05)。阴道超声子宫内膜≤1.5mm与子宫内膜血流RI检出率分别为97.44%、96.15%比腹部超声71.79%、61.23%更高(P<0.05)。结论和腹部超声比较,予以异位妊娠患者阴道超声检查的诊断准确率更高,同时简便、快捷,可以减少漏诊和误诊现象出现,值得采用。

Objective To analyze the diagnostic value of transvaginal ultrasound and transabdominal ultrasound in ectopic gestation.Methods Clinical data about 78 patients of being surgically and pathologically confirmed with ectopic gestation from January 2019 to December 2019 in our hospital was retrospectively analyzed.Abdominal sonography and then vaginal sonography were performed.Diagnosis accuracy,imaging manifestations,time of displaying gestational sac and confirmation time,endometrial thickness and endometrial blood flow RI(resistance index)detection conditions were compared.Results Diagnosis accuracy based on vaginal sonography(97.44%)was significantly higher than that of abdominal sonography(82.05%)(P<0.05).Detection rates of pelvic effusion,phantom pregnancy bursa,adnexal masses,cardiovascular pulsation and embryo based on vaginal sonography(92.31%,19.23%,97.44%,17.95%,20.51%)were significantly higher than those of abdominal sonography(64.10%,3.85%,82.05%,3.85%,5.13%)(P<0.05).Time of displaying gestational sac and confirmation time based on vaginal sonography was(42.85±5.96)d and(45.48±6.75)d,which were significantly shorter than those of abdominal sonography[(52.50±6.18)d,(56.35±7.24)d](P<0.05);the detection rates of endometrial thickness≤1.5mm and endometrial blood flow RI based on vaginal sonography(97.44%,96.15%)were significantly higher than those of abdominal sonography(71.79%,61.23%)(P<0.05).Conclusion Compared with abdominal sonography,vaginal sonography is featured as high diagnosis accuracy,easy and fast operation,and low rate of missed diagnosis and misdiagnosis.It is worthy of application.
