肺腺癌患者EGFR基因突变与血清肿瘤标志物CEA的相关性研究

Relationship between EGFR Gene Mutation and Serum Tumor Marker CEA in Patients with Lung Adenocarcinoma

目的探究肺腺癌患者表皮生长因子受体(EGFR)基因突变与血清肿瘤标志物血清癌胚抗原(CEA)的相关性。方法从我院选取2018年6月至2020年6月接收治疗的肺腺癌患者54例为本次实验的调查研究对象,运用临床技术手段检测表皮生长因子受体(EGFR)基因突变情况、血清肿瘤标志物血清癌胚抗原(CEA)水平,探究肺腺癌患者表皮生长因子受体(EGFR)基因突变与血清肿瘤标志物血清癌胚抗原(CEA)的相关性。结果本次实验共检测出50例EGFR基因突变患者,EGFR突变只发生在第18~21外显子,其中第19外显子的框内缺失性突变和第21外显子的L858R替代突变约占所有EGFR突变的80%以上;女性表皮生长因子受体阳性率高于男性;本次调查显示血清癌胚抗原表达水平异常(≥5 ng/mL)时表皮生长因子受体阳性率高于血清癌胚抗原表达水平正常(<5 ng/mL)时;表皮生长因子受体基因阳性突变率随血清癌胚抗原浓度增高而上升;经Spearman相关性分析,发现表皮生长因子受体基因突变概率与血清癌胚抗原表达水平呈正相关。结论肺腺癌患者表皮生长因子受体基因突变概率与血清癌胚抗原表达水平呈正相关,可在一定程度上预测表皮生长因子受体基因突变状况,监测血清癌胚抗原表达水平可为临床治疗提供参考资料。

Objective To explore the correlation between epidermal growth factor receptor (EGFR) gene mutation and serum tumor marker serum carcinoembryonic antigen (CEA) in patients with lung adenocarcinoma.Methods Fifty-four patients with lung adenocarcinoma received treatment from June 2018 to June 2020 were selected as the subjects of this study.The mutation of EGFR gene,serum carcinoembryonic antigen (CEA) level of tumor markers were detected by clinical technique,and the correlation between EGFR gene mutation and CEA were studied.Results In this study,50 patients with EGFR gene mutation were detected.EGFR mutation occurred only in exon 18~21,among which 19 exon intrabox deletion mutation and L858R substitution mutation of exon 21 accounted for more than 80% of all EGFR mutations.The positive rate of EGFR in female was higher than that in male.In this investigation,the positive rate of EGFR in abnormal serum CEA expression level (≥5 ng/mL) was higher than that in normal serum CEA expression level (<5ng/mL).The positive mutation rate of EGFR gene increased with the increase of serum CEA concentration.Spearman correlation analysis showed that the mutation probability of EGFR gene was positively correlated with CEA expression level.Conclusion There is a positive correlation between the mutation probability of EGFR gene and the expression level of serum carcinoembryonic antigen in patients with lung adenocarcinoma.To a certain extent,it can predict the mutation of epidermal growth factor receptor gene.Monitoring the expression level of serum carcinoembryonic antigen can provide reference for clinical treatment.
