中老年高血压性脑出血患者血肿破入脑室的治疗及临床效果

The Treatment and Clinical Effect of Hematoma Breaking into Ventricles in the Elderly Patients with Hypertensive Intracerebral Hemorrhage

目的分析中老年高血压性脑出血患者血肿破入脑室的治疗及临床效果。方法将我院30例的中老年高血压脑出血患者作为本次研究对象,随机分成两组,对照组15例进行保守治疗,观察组15例进行大骨瓣开颅血肿清除术治疗,对比两组临床效果。结果观察组的病死率为13.33%(2/15)、对照组的病死率为40.00%(6/15),两者比较差异有统计学意义(P<0.05);对照组的并发症的发生率明显低于观察组,两者比较差异有统计学意义(P<0.05);在两组进行治疗后,观察组的GOS评分明显高于对照组,对照组的MRS评分以及NIHSS评分明显高于观察组,两者比较差异有统计学意义(P<0.05)。结论为中老年高血压性脑出血患者采取大骨瓣开颅血肿清除术治疗,具有安全性较高、预后效果较好等特点,能有效降低患者的死亡率、并发症的发生率,神经功能恢复效果较好,让临床疗效显著提高,值得临床推广。

Objective To analyze the treatment and clinical effect of hematoma breaking into ventricles in middle-aged and old patients with hypertensive cerebral hemorrhage.Methods Thirty middle-aged and old patients with hypertensive cerebral hemorrhage in our hospital were randomly divided into two groups.Fifteen cases in the control group were treated conservatively,and 15 cases in the observation group were treated with large bone flap craniotomy to remove hematoma.Results The mortality of the observation group was 13.33%(2/15)and that of the control group was 40.00%(6/15),the difference was statistically significant(P<0.05).The incidence of complications in the control group was significantly lower than that in the observation group,the difference was statistically significant(P<0.05).After the treatment of the two groups,the GOS score in the observation group was significantly higher than that in the control group,the Mrs score and NIHSS score in the control group were significantly higher The score of the two groups was significantly higher than that of the observation group(P<0.05).Conclusion For the middle-aged and old patients with hypertensive intracerebral hemorrhage,large bone flap craniotomy for hematoma removal has the characteristics of high safety and good prognosis,which can effectively reduce the death rate and the incidence of complications,and the effect of neurological function recovery is good,so that the clinical effect is significantly improved,which is worthy of clinical promotion.
