心脏介入诊疗术后并发心包填塞的临床观察和护理对策

Clinical Observation and Nursing Countermeasures of Cardiac Tamponade after Cardiac Intervention

目的探讨心脏介入诊疗术后并发心包填塞临床观察的要点及护理对策。方法回顾分析我院近半年来心脏诊疗介入术,包括射频消融术、PCI术后并发心包填塞的6例患者的救治过程及护理,总结出临床观察要点及护理措施。结果6例患者均予心包穿刺,1例患者穿刺后抽出100毫升积液后置管失败,予补液扩容止痛等对症治疗后生命体征平稳。其余5例均予穿刺抽液后留置心包引流管引流,症状迅速缓解,血流动力学稳定。结论严密的临床观察,尽早识别,快速给予输液、输血,配合心包穿刺及有效的护理措施,是成功抢救心包填塞并发症的关键。

Objective To explore the main points of clinical observation and nursing measures of pericardial tamponade after cardiac interventional treatment.Methods Retrospective analysis,in our hospital in the past six months of cardiac interventional treatment.Treatment and nursing of six patients with pericardial tamponade after radiofrequency ablation and PCI.Summarize the clinical observation points and nursing measures.Results All the six patients were treated with pericardiocentesis.In one patient,100mL of effusion was extracted after puncture,and catheterization failed.After symptomatic treatment,such as fluid supplementation,volume enlargement and analgesia,the vital signs were stable.In the other five cases,pericardial drainage was performed with indwelling pericardial drainage tube after pericardial drainage.Conclusion Strict clinical observation,early identification,rapid infusion,blood transfusion,pericardial puncture and effective nursing measures are the keys to the successful rescue of pericardial tamponade complications.
