超声引导细针穿刺细胞学检查对TI-RADS4类甲状腺结节的价值

The Value of Ultrasound-guided Fine-needle Aspiration Cytology in TI-RADS4 Thyroid Nodules

目的针对超声引导细针穿刺细胞学检查对TI-RADS4类甲状腺结节的价值进行研究探讨。方法选取本院在2019年收治的TI-RADS4类甲状腺结节患者71例,进行超声引导穿刺细胞学检查,分为A组、B组、C组进行实验观察,针对患者在超声引导后的FANB特异度、阴性预测值、灵敏度、阳性预估值进行计算。结果A组患者Kappa值为0.85,B组Kappa值为0.76,C组Kappa值为0.81,三组Kappa值皆>0.75;A组的特异度为90.5%,阴性预测值为87.1%,灵敏度为87.4%,阳性预估值为97.1%;B组特异度为94.3%,阴性预测值为89.6%,灵敏度为88.4%,阳性预估值为96.3%;C组特异度为94.7%,阴性预测值89.5%,灵敏度为94.7%,阳性预估值为92.4%。结论超声引导细针穿刺细胞学检查对TI-RADS4类甲状腺结节的临床诊断具有较高的准确性,值得临床借鉴使用。

Objective To study the value of ultrasound-guided fine-needle aspiration cytology in TI-RADS4 thyroid nodules.Methods Seventy-one patients with TI-RADS4 thyroid nodule in our hospital in 2019 were divided into A group,B group and C group for experimental observation.The FANB specificity,negative predictive value,sensitivity and positive predictive value were calculated.Results The kappa value of group A was 0.85,that of group B was 0.76,that of group C was 0.81,and that of three group>0.75.The specificity of group A was 90.5%,the negative predictive value was 87.1%,the sensitivity was 87.4%,and the positive predictive value was 97.1%.The specificity of group B was 94.3%,the negative predictive value was 89.6%,the sensitivity was 88.4%,and the positive predictive value was 96.3%.The specificity of group C was 94.7%,and the negative predictive value was 89.5%,the sensitivity was 94.7%,and the positive predictive value was 92.4%.Conclusion Ultrasound-guided fine-needle aspiration cytology has a high accuracy in the clinical diagnosis of TI-RADS4 thyroid nodules.It is worthy of clinical reference.
