腹腔镜下直肠癌低位前切除术中保留左结肠动脉的可行性及临床价值

Feasibility and Clinical Value of Preserving Left Colon Artery in Laparoscopic Low Anterior Resection of Rectal Cancer

目的评价腹腔镜下直肠癌低位前切除术中保留左结肠动脉的可行性及临床价值。方法将2016年9月至2018年1月在我院行腹腔镜下直肠癌低位前切除术的76例患者根据术中是否保留左结肠动脉分为研究组、对照组,对两组患者的临床疗效对比分析。结果两组患者的手术用时、术中出血量、清扫淋巴结数量比较,P>0.05,无统计学意义。研究组术后并发症发生率较对照组低,P<0.05,有统计学意义,术后随访18个月期间的局部复发率和远处转移率与对照组比较,P>0.05,无统计学意义。结论腹腔镜下直肠癌低位前切除术中保留左结肠动脉有利于减少患者术后并发症的发生,具有可行性。

Objective To evaluate the feasibility and clinical value of preserving left colon artery in laparoscopic anterior resection of rectal cancer.Methods 76 patients who underwent laparoscopic anterior resection of rectal cancer from September 2016 to January 2018 were divided into study group and control group according to whether the left colon artery was preserved during the operation.The clinical efficacy of the two groups were compared and analyzed.Results Compared with the operation time,intraoperative blood loss and lymph node dissection number of the two groups,P>0.05,with no statistical significance.The incidence rate of postoperative complications in the study group is lower than that in the control group(P<0.05),which is statistically significant.the local recurrence rate and distant metastasis rate in the 18-month follow-up period after surgery are all higher than that in the control group(P>0.05),which is not statistically significant.Conclusion It is feasible to preserve the left colon artery in laparoscopic anterior resection of rectal cancer for reducing postoperative complications.
