疑难交叉配血产生的原因及意外抗体的分布特点

The Causes of Difficult Cross Matching and the Distribution Characteristics of Unexpected Antibodies

目的探讨疑难交叉配血产生的原因及意外抗体的分布状况。方法回顾分析2016年4月至2018年12月收治的交叉配血不相合和抗体筛查结果为阳性的患者120例,输血前均接受相应检查,分析疑难交叉配血出现的主要原因和意外抗体分布特征。结果抗体筛选共113例阳性(94.17%),共7例抗体筛选阴性(5.83%)。阳性病例中,同种抗体为64例(53.33%),其次为32例自身温抗体(26.67%)。抗体鉴定阳性标本113例,共68例有特异性者(60.18%);导致抗体筛选为阴性,且发生交叉配血不相合的原因为血型未正确定型。结论造成疑难交叉配血产生的主要原因之一则为存在意外抗体,因此输血前做抗体筛查具有必要性。

Objective To explore the causes of difficult cross matching and the distribution of unexpected antibodies.Methods A retrospective analysis was made of 120 patients who were admitted from April 2016 to December 2018 with mismatched cross matching and positive antibody screening results.Before transfusion,all patients were examined accordingly,the main causes of difficult cross matching and the distribution characteristics of unexpected antibodies were analyzed.Results One hundred and thirteen cases were positive(94.17%)and 7 cases were negative(5.83%).Among the positive cases,64(53.33%)were Isoantibodies,followed by 32(26.67%)autoantibodies.There were 113 positive samples of antibody identification,68 of which had specificity(60.18%).The cause of antibody screening was negative,and the cause of cross matching was that the blood type was not correctly determined.Conclusion One of the main causes of difficult cross matching is the presence of unexpected antibodies,so it is necessary to do antibody screening before blood transfusion.
